摘要
目的探讨血管内皮损伤生物标志物[细胞间黏附分子1(ICAM-1)、内皮素1(ET-1)]联合载脂蛋白A1(ApoA1)、核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)检测对青年痛风的诊断价值。方法选取2017年8月至2020年8月徐州医科大学附属淮海医院收治的123例青年痛风患者作为病例组(慢性痛风69例、急性痛风54例);另外,选取同期在徐州市医科大学附属淮海医院的体检健康者60例作为对照组。比较两组临床资料及血清细胞间黏附分子1(ICAM-1)、内皮素1(ET-1)、ApoA1、NALP3水平;比较急、慢性痛风患者血清ET-1、ICAM-1、ApoA1、NALP3水平。采用Spearman相关分析血清ET-1、ICAM-1、ApoA1、NALP3与青年痛风类型的相关性;采用Logistic回归分析青年痛风的危险因素;采用受试者工作特征曲线(ROC)分析血清ET-1、ICAM-1、ApoA1、NALP3对青年痛风的诊断价值。结果病例组有饮酒史、高嘌呤饮食的比例及血清ET-1、ICAM-1、NALP3水平显著高于对照组(P<0.05)。病例组血清ApoA1水平显著低于对照组(P<0.05)。急性痛风患者血清ET-1、ICAM-1、NALP3显著高于慢性痛风患者(P<0.05)。急性痛风患者血清ApoA1显著低于慢性痛风患者(P<0.05)。有饮酒史、高嘌呤饮食、血清ET-1高水平、血清ICAM-1高水平、血清NALP3高水平、血清ApoA1低水平是青年痛风的危险因素(P<0.05)。血清ET-1、ICAM-1、NALP3水平增高与急性痛风的发生相关(P<0.05)。血清ApoA1水平的增高与慢性痛风的发生相关(P<0.05)。血清ET-1、ICAM-1、ApoA1、NALP3联合诊断青年痛风的曲线下面积(AUC)为0.978,灵敏度为92.70%,特异度为95.00%,准确度为91.69%,截断值依次为77.82 pg/mL、534.44μg/L、0.87 mmol/L、6.44 ng/mL。结论血管内皮损伤生物标志物ET-1、ICAM-1联合ApoA1、NALP3诊断青年痛风的价值较高。
Objective To investigate the diagnostic value of combined detection of vascular endothelial injury biomarkers[intercellular adhesion molecule 1(ICAM-1),endothelin 1(ET-1)]combined with apolipoprotein A1(ApoA1)and nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)for gout in young people.Methods A total of 123 young patients with gout in our hospital from August 2017 to August 2020 were enrolled in the study as patients group,including 69 patients with acute gout and 54 patients with chronic gout,meanwhile,60 healthy individuals were enrolled as control group.The clinical data and the levels of serum ICAM-1,ET-1,ApoA1 and NALP3 were compared between the two groups and the acute and chronic gout patients.Spearman correlation analysis was used to verify the correlation between serum ET-1,ICAM-1,ApoA1,NALP3 and the type of gout in young people;Logistic regression was used to screen the risk factors of youth gout;the receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of serum ET-1,ICAM-1,ApoA1,and NALP3 for gout in yong people.Results The proportion of people who had drinking history and high purine diet,and serum levels of ET-1,ICAM-1 and NALP3 in patients group were significantly higher than those in control group(P<0.05).Serum levels of ApoA1 in patients group were significantly lower than those of patients group(P<0.05);compared with chronic gout patients,acute gout patients had significantly higher levels of ET-1,ICAM-1 and NALP3,but lower levels of ApoA1(P<0.05).Drinking history,high-purine diet,elevated levels of serum ET-1,ICAM-1 and NALP3,and low levels of serum ApoA1 were risk factors for gout in young people(P<0.05).Serum ET-1,ICAM-1 and NALP3 levels were correlated with acute gout(P<0.05);serum ApoA1 level was correlated with chronic gout(P<0.05).The area under curve(AUC),sensitivity,specificity and accuracy of combined detection of serum ET-1,ICAM-1,ApoA1 and NALP3 in the diagnosis of gout were 0.978,92.70%,95.00%and 91.69%,respectively.The cut-off v
