摘要
目的:评价高强度聚焦超声(HIFU)睫状体成形术治疗闭角型青光眼的效果。方法:回顾性分析郑州大学第一附属医院2018年12月至2020年5月接受HIFU睫状体成形术治疗各种类型的闭角型青光眼32例(32只眼)的临床资料,术后随访6个月。观察术后各时间点视力、眼压、降眼压药物数量、前房角的变化和并发症发生率。结果:术后各时间点视力(logMAR)与术前1.76±0.98相比差异无统计学意义( F=0.176, P=0.983),术后眼压逐渐下降( F=85.903, P<0.001)。随访期末,32只眼中手术完全成功者19例,部分成功者10例,失败者3例,成功率为90.63%(29/32)。术后3个月的前房角各参数(前房深度、前房角开放距离、小梁虹膜夹角、及虹膜内表面与晶状体前表面的夹角)与术前各参数相比差异均无统计学意义( P>0.05)。手术并发症包括角膜水肿者10只眼(31.25%)、结膜充血者24只眼(75.00%)、一过性高眼压者2只眼(6.25%);未出现脉络膜上腔出血、脉络膜脱离或眼内炎等严重的并发症。 结论:高强度聚焦超声睫状体成形术是闭角型青光眼的有效疗法。
Objective To evaluate the efficacy of high intensity focused ultrasound cycloplasty for the treatment of angle-closure glaucoma.Methods The clinical data of 32 eyes of 32 patients with various types of angle-closure glaucoma treated with high intensity focused ultrasound(HIFU)cycloplasty from Dec.2018 to May 2020 in the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed.The postoperative follow-up time was 6 months.The visual acuity,intraocular pressure,anti-glaucoma medicines,anterior chamber angle and complications at postoperative time points were observed.Results There was no statistically significant difference in visual acuity at preoperative time points and postoperatively(F=0.176,P=0.983).The intraocular pressure decreased gradually after surgery(F=85.903,P<0.001).At the end of the follow-up time,surgery was completely successful in 19 cases,partially successful in 10 cases,and failed in 3 cases.The success rate was 90.63%(29/32).There were no statistically significant differences in each parameters of anterior chamber angle such as anterior chamber depth,anterior chamber angle opening distance,trabecular-iris angle,the angle between inner surface of iris and anterior surface of lens between preoperatively and 3 months postoperatively(P>0.05).Surgical complications included corneal edema in 10 eyes(31.25%),conjunctival congestion in 24 eyes(75.00%)and transient intraocular hypertension in 2 eyes(6.25%).No serious complication such as suprachoroidal hemorrhage,choroidal detachment or endophthalmitis occurred.Conclusion High intensity focused ultrasound cycloplasty is an effective treatment for angle-closure glaucoma.
