摘要
目的探讨隧道式经股静脉PICC置管患者的真实体验,为临床制定心理干预措施提供参考依据。方法采用质性研究中的现象学研究法,对13名隧道式经股静脉PICC置管患者进行半结构式访谈,并采用Colaizzi 7步分析法进行资料分析。结果隧道式经股静脉PICC置管患者置管前、置管中以负性体验为主,主要表现为:紧张、恐惧、羞涩、尴尬、担心与无奈;置管后主要以正性体验为主,主要表现为:放松与欣慰,安全感。结论针对患者置管全程的心理变化,提示护理人员应提高患者隐私保护意识,加强置管全程的健康教育和人文关怀,提高患者相关知识认知度和心理舒适度,同时加强专科护士培训和置管前血管通路的评估,提高置管成功率,从而激发患者的正性体验、疏导负性心理体验,提高患者满意度。
Objective To explore the real experience of patients with PICC through femoral vein,and to provide reference for clinical psychological intervention.Methods With phenomenological research method,semi-structured interviews were conducted in 13 patients with PICC via femoral vein,and the data were analyzed by the 7-step method of Colaizzi.Results Before and during the catheterization of PICC,the negative experience dominated the patients and main manifestations were tension,fear,shyness,embarrassment,worry and helplessness.After the placement of the catheter,patients majorly had the positive experience and the main manifestations were relaxation and comfort,and sense of security.Conclusion According to the psychological changes of patients during the whole process of intubation,the protection of patients’privacy,health education and humanistic care,the training of specialized nurses and the assessment of vascular access before catheterization are conducive to improving the success rate of intubation,stimulating the positive experience of patients and improving patient satisfaction.
