摘要
目的探讨左肝外叶肝胆管结石患者实施腹腔镜肝切除术治疗的应用效果。方法回顾性分析2016年1月至2021年1月我院收治的51例有胆道手术史左肝外叶胆管结石手术患者临床资料,根据手术方式分成腹腔镜组(n=24例)与开腹组(n=27例),对比观察两组治疗效果。结果腹腔镜组患者手术时间、术中出血量、胆漏、术后出血与开腹组相比,无统计学意义(P>0.05)。腹腔镜组在住院时间、排气时间、术后切口感染、术后24 h对免疫功能的影响方面与开腹组相比,有统计学意义(P<0.05)。结论有胆道手术史的左肝外叶胆管结石患者实施腹腔镜肝切除术治疗法能够缩短患者的住院时间,减少对患者免疫功能的影响,安全有效。
Objective To observe the effect of laparoscopic hepatectomy in the treatment of left lateral lobe hepatolithiasis.Methods From January 2016 to January 2021,the clinical data of 51 patients with bile duct operation history of left lateral lobe bile duct stones in our hospital were retrospectively analyzed.According to the operation methods,they were divided into laparoscopic group(n=24 cases)and open group(n=27 cases),and the therapeutic effects of the two groups were compared and observed.Results The operation time,intraoperative blood loss,bile leakage and postoperative bleeding of laparoscopic group were significantly shorter than those of control group;compared with the open group,there was no statistical significance(P>0.05).Compared with the open group,in the laparoscopic group,the hospitalization time,exhaust time,postoperative incision infection,the effect of postoperative immune function was statistically significant(P<0.05).Conclusion Laparoscopic hepatectomy for patients with bile duct stones in the left extrahepatic lobe with a history of biliary surgery can shorten the patient's hospital stay,reduce the impact on the patient's immune function,and is safe and effective.
