摘要
目的分析血液透析联合血液透析滤过对糖尿病肾病患者肾功能与心功能的影响。方法随机选取医院2018年1月-2020年1月收治的DN患者220例,使用计算机随机数字分组法将其等分成对照组与观察组,每组110例。对照组患者使用血液透析治疗,观察组患者使用血液透析联合血液透析滤过治疗。比较两组患者肾功能、心功能、磷钙水平和健康状态。结果透析前两组患者肾功能相近,前SCr、BUN、CCr、BNP水平差异比较无统计学意义(P>0.05),透析后降低透析前(P<0.05);观察组患者SCr、BUN、CCr水平低于对照组(P<0.05)。透析前两组患者心脏功能相同,LVEDD、LVPWDT、LVEF差异比较无统计学意义(P>0.05),透析后LVEDD、LVPWDT降低(P<0.05),LVEF增加(P<0.05);观察组患者LVEDD、LVPWDT低于对照组(P<0.05),LVEF高于对照组(P<0.05)。透析前两组患者磷钙水平相似,血磷、血钙、钙磷乘积差异比较无统计学意义(P>0.05),透析后血磷、血钙、钙磷乘积低于透析前(P<0.05);观察组患者血磷、血钙、钙磷乘积低于对照组(P<0.05)。观察组患者600ml/d时间,APACHEⅡ评分低于对照组(P<0.05)。结论DN患者接受血液透析联合血液透析滤过治疗,能获得更好的治疗效果,建议使用。
Objective To investigate the impact of hemodialysis combined with hemodiafiltration on renal function and cardiac function in maintenance hemodialysis patients with diabetic nephropathy(DN).Methods 220 DN patients in our hospital from January 2018 to January 2020 were enrolled,and divided into two groups by random number table methods,each with 110 cases.Control group received hemodialysis,while observation group received hrmodialysis combined with hemodiafiltration.Renal function,cardiac function,phosphorus and calcium levels and health status were compared between groups.Results The renal function related indexes SCr,BUN,CCr and BNPhad no significant difference between groups before treatment(P>0.05),which were decreased in both groups after treatment(P<0.05),and the decrease was more significant in observation group than in control group(P<0.05).The cardiac function related indexes LVEDD,LVPWDT,and LVEF had no significant difference between groups before treatment(P>0.05).After treatment,there was a decrease in LVEDD and LVPWDT,and an increase in LVEF in both groups(P<0.05),moreover,both changes were more obvious in observation group than in control group(P<0.05).The phosphorus and calcium levels were similar in two groups,and no significant difference was found in blood phosphorus,blood calcium,calcium and phosphorus(P>0.05).After treatment,blood phosphorus,blood calcium,calcium and phosphorus were decreased in both groups(P<0.05),and were lower in observation group than in control group(P<0.05).The APACHEⅡscore of observation group was lower than that of control group in terms of urine output<600 ml/d(P<0.05).Conclusion Hemodialysis combined with hemodiafiltration has good treatment effect for DN patients,which is worthy of recommendation.
