摘要
目的分析正常胎儿肺肝信号比(LLSR)与孕周关系,进一步探讨LLSR在产前诊断的临床应用价值。方法回顾性收集515例于郑州大学第三附属医院行产前MRI检查的孕21^(+3)~41^(+5)周胸部正常胎儿资料,在T 2WI冠状面图像上测得LLSR,对各个孕周LLSR进行统计描述并探讨LLSR与孕周之间的关系,建立回归模型,通过绘制受试者工作特征曲线(ROC)得出区分孕周<28周与孕周≥28周的最佳临界值。结果515例胎儿平均LLSR(2.39±0.83),LLSR与孕周呈正相关关系(r=0.691,P<0.05),回归方程为Y=-0.573+0.097X(Y为LLSR,X为孕周,R^( 2)=0.721,P<0.01);当LLSR取值2.03时,具有区分孕周<28周与孕周≥28周的最佳诊断效能,诊断孕周≥28周的敏感度为78.82%,特异度为79.43%,假阳性率为20.57%,假阴性率为21.18%。结论LLSR与孕周呈线性相关关系,并得出区分孕周是否满28周的最佳临界值,可为产前诊疗提供很大临床应用价值。
Objective To analyze the relationship between lung-to-liver signal intensities ratio(LLSR)and gestational week in a large sample of normal fetuses,and further explore the clinical application value of LLSR in prenatal diagnosis.Methods The data of 515 pregnant women of 21^(+3)to41^(+5) who underwent MRI in the Third Affiliated Hospital of Zhengzhou University with normal lungs were retrospectively analyzed.The LLSR was calculated from the T 2WI coronal images.LLSR per gestational week was described and regression analysis was used to analyze the relation between the LLSR and gestational week.The ROC curve was drawn to determine the optimal threshold to distinguish between gestational week less than 28 weeks and no less than 28 weeks.Results Summary statistics for LLSR of 515 cases showed a mean value of 2.39±0.83.LLSR was positively correlated with gestational week(r=0.691,P<0.05).Linear regression formula was Y=-0.573+0.097X(Y=LLSR,X=gestational week,R^( 2)=0.721,P<0.01).The optimal threshold for differentiating fetal lungs with gestational week no less than 28 weeks from less than 28 weeks was 2.03,with sensitivity,specificity,false positive and false negative of 78.82%,79.43%,20.57%,and 21.18%,respectively.Conclusion There is a linear correlation between LLSR and gestational week,and the optimal threshold value is obtained to distinguish whether the gestational week was over 28 weeks,which could provide great clinical application value for prenatal diagnosis and treatment.
