摘要
目的了解携带ermB耐药基因肺炎链球菌(S.pn)的血清型分布及多位点序列分型(MLST)状况,为疫苗免疫预防的可行性及菌株遗传背景研究提供参考依据。方法复苏2018年1月-2019年6月杭州市西湖区中西医结合医院保存儿科下呼吸道感染患者分离的肺炎链球菌,采用E-test法检测分离株对青霉素、头孢噻肟、头孢曲松的最低抑菌浓度(MIC),并采用梅里埃VITEK 2 Compact全自动细菌鉴定及药敏分析系统检测美罗培南等其他12种抗菌药物的敏感性;应用聚合酶链式反应(PCR)进行血清型鉴定、多位点序列分型(MLST)及大环内酯类耐药基因ermB检测,根据年龄不同,将患儿分为3组(0~12月,13~36月与37~60月)。结果筛查出59株携带ermB耐药基因S.pn作为研究对象:从中共检出7种血清型,以血清型3,19A及6A/B为主,构成比分别为37.29%、16.95%和15.25%;ST序列国际流行克隆菌株以ST271与ST505为主,构成比均为15.25%,ST271血清型主要以19A为主,菌株多重耐药百分比为44.44%;ST505血清型均为3;菌株多重耐药百分比为88.88%,非国际流行克隆菌株以ST902与ST6989为主,构成比分别为6.98%与5.08%,ST902血清型主要为6A/B,多重耐药菌株百比为50.00%,ST6989血清型均为3,多重耐药菌株百分比为66.67%。结论该地区S.pn以血清型3及19A为主,多位点序列分型国际流行克隆菌株以ST505与ST271为主,非国际流行菌株以ST902与ST6989为主。
Objective To investigate the serotype distribution and multi-locus sequence typing(MLST)of Streptococcus pneumoniae(S.pneumococcus)carrying ermB drug-resistant gene,and to provide references for the feasibility of vaccine immunization and genetic hereditary background research.Method Streptococcus pneumoniae isolated from the clinical laboratory of our hospital from January,2018 to June,2019 were resuscitated.The minimum inhibitory concentration(MIC)of the isolates to penicillin,cefotaxime and ceftriaxone was detected by E-test method,and the susceptibility of meropenem,and other 12 antibiotics were detected by Merieux Vitek 2 Compact automatic bacterial identification and drug sensitivity analysis system.Serotype identification,multi-locus sequence typing(MLST)and ermB detection were performed by polymerase chain reaction(PCR).The children were divided into three groups(0 to 12 months,13 to 36 months and 37 to 60 months)according to their ages.Result A total of 59 strains of S.pn carrying ERMB drug resistance gene were screened as the object of this study.A total of 7 serotypes were detected,and serotypes of 3,19A and 6A/B were the main ones,with composition ratios of 37.29%,16.95%and 15.25%,respectively.ST271 and ST505 were the main cloned strains of ST sequence in the international epidemic,and the composition ratio was 15.25%.The serotype of ST271 was mainly 19A,and the percentage of multiple drug resistance was 44.44%,and the serotype of ST505 was 3.The percentage of multi-drug resistant strains was 88.88%,and the non-internationally prevalent clone strains were ST902 and ST6989 with composition ratios of 6.98%and 5.08%respectively.The serotype of ST902 was mainly 6A/B and the percentage of multi-drug resistant strains was 50.00%,as well as the serotype of ST6989 was 3 and the percentage of multi-drug resistant strains was 66.67%.Conclusion Serotype 3 and 19A are the main S.pn types in this area.The international epidemic clones were mainly ST505 and ST271,and the non-international epidemic strains were main
