摘要
目的探讨本院住院药房药品信息闭环流程模块化改造的实践效果。方法介绍本院住院药房药品调剂信息闭环化改造的建设情况,收集流程改造前后的运行数据,评价住院药房改造前后的调剂模式及成效;调剂流程改造中引入多模块化管理理念设计出适用于不同病区的3种调剂进阶流程,评价模块化理念对住院药房调剂工作模式多样性的意义。结果本院在经过药品调剂信息闭环化改造后,住院药房差错率大幅下降,整体调剂效率相对提高。改造后相对于改造前同期内差由0.25%下降至0.026%,人均每日调配耗时下降9.14%,单条医嘱调配耗时下降4.19%;进阶调剂流程评价结果:①内差数(进阶调剂流程1>进阶调剂流程2>进阶调剂流程3);②单条医嘱均耗时(进阶调剂流程2>进阶调剂流程1>进阶调剂流程3)。结论本院药品调剂流程信息闭环改造后,改变了传统的药品调剂模式,大幅降低差错率,3种模块调剂流程较好地适应了不同病区的需求,提高了整体调剂效率,实现从医嘱开具到患者用药的大闭环。
Objective To evaluate the modularized transformation of close-loop management of medication in the inpatient pharmacy of our hospital.Methods The medication close-loop transformation in the inpatient pharmacy was introduced.Working models and performance before and after the transformation data were analyzed and compared.Three drug dispensing processs were designed by multi-module management to make it suitable for different inpatient wards.The multi-module management in the drug dispensing was evaluated.Results After the close-loop modification the error rate of drug dispensing was decreased significantly,and the overall dispensing efficiency improved.The error rate was reduced from 0.25%to 0.026%,the daily allocation time per capita was reduced by 9.14%,and the allocation time for each order was reduced by 4.19%.Evaluation of 3 advanced transfer processes:①error rate(process 1>process 2>process 3);②time of dispensing for each order(process 2>process 1>process 3).Conclusion After the modification of medication close-loop management in the inpatient pharmacy,the traditional working model of drug dispensing is changed,and error rate is greatly reduced.Various modularized drug dispensing processes help adapt to the needs of different wards,improve the overall dispensing efficiency,and realize the close-loop from doctors’order to patients’medication.
