摘要
目的:观察深刺夹脊穴至神经根联合手法推拿治疗腰椎间盘突出症对临床症状、血液流变学影响。方法:选取2019年1月~2020年5月收治的83例腰椎间盘突出症患者为研究对象,根据随机数字表法分为对照组(42例/组)和实验组(41例/组),对照组给予西医对症治疗,实验组给予深刺夹脊穴至神经根联合手法推拿治疗,比较两组临床疗效,中医证候积分、腰痛、腰椎活动功能、血流动力学指标及生活质量。结果:实验组治疗7d后临床有效率高于对照组(P<0.05);实验组治疗7d后痛处拒按、活动受限、下腰痛、腰部板硬等证候积分均低于对照组(P<0.05);实验组治疗7d后日本骨科协会下腰痛量表(M-JOA)、腰椎Oswestry功能障碍指数(ODI)评分均低于对照组(P<0.05);实验组治疗7d后全血粘度、血浆粘度均低于对照组(P<0.05);实验组治疗7d后生理机能、生理职能、社会功能、情感职能、活力、疼痛、精神健康、总体健康变化等简明健康状况调查表(SF-36)评分高于对照组(P<0.05)。结论:深刺夹脊穴至神经根联合手法推拿治疗腰椎间盘突出症的效果突出,可显著缓解中医症状,减轻腰痛、腰椎活动障碍和改善血液循环,显著提升患者生活质量。
Objective:To observe the effect of applying deep needling Jiaji acupoint to nerve root combined with massage manipulation in the treatment of lumbardisc herniation(LDH)on clinical symptoms and hemorheology.Methods:A total of 83 LDH patients admitted from January 2019 to May 2020 were enrolled as the research objects.According to random numbeRtable method,they were divided into a controlled group(42 cases)and an experimental group(41 cases).The group was given symptomatic treatment with western medicine,while the experimental group was given deep needling Jiaji acupoint to nerve root combined with massage manipulation.The clinical curative effect,scores of TCM syndromes,backache,lumbaRactivity function,hemodynamic indexes and quality of life were compared between the two groups.Results:AfteR7d of treatment,clinical response rate in the experimental group was higheRthan that in the controlled group(P<0.05),scores of symptoms such as refusal to pain press,movement limitation,low back pain,and lumbar rigidity were lower than those in the controlled group(P<0.05),scores of Modified Japanese Orthopaedic Association(M-JOA)and Oswestry Dysfunction Index(ODI)were lower than those in the controlled group(P<0.05),whole blood viscosity and plasma viscosity were lower than those in the controlled group(P<0.05),and scores of physiological function,physiological role,social function,emotional role,vitality,pain,mental health and overall health change in 36-item shot-form health status survey(SF-36)were higher than those in the controlled group(P<0.05).Conclusion:The curative effect of deep needling Jiaji acupoint to nerve root combined with massage manipulation is significant on LDH.It can significantly alleviate TCM symptoms,backache and lumbar dysfunction,improve blood circulation,and significantly improve quality of life in patients.
