摘要
目的研究膝关节内翻性骨关节炎患者胫骨高位截骨TomoFix锁定钢板固定术后对患者骨关节功能的影响及临床疗效的研究。方法膝关节内翻性骨关节炎的患者40例,应用2000 IKDC对患者术前、术后的功能状况进行评价,同时对膝关节内翻性骨关节炎患者胫骨高位截骨TomoFix锁定钢板固定术后采用Lysholm评分、2000 IKDC评分、Cincinnati评分、简明健康状况量表(SF-36)评价及Tegner、Lysholm运动水平评级,根据患者体重指数(BMI)、年龄、性别进行分组并对其结果差异进行分析;应用2000 IKDC评分评价患者疼痛指标及分析胫骨高位截骨TomoFix锁定钢板固定术对膝关节内翻性骨关节炎的临床改善情况。结果术后2000 IKDC评分较术前明显升高(P<0.05);术后患者疼痛明显改善,且术后1个月时所有患者疼痛程度均可接受;SF-36评价术后生理职能得分较术前明显升高;SF-36心理职能术后得分较术前明显升高;Tegner、Lysholm评级自术前3级升至6级;将年龄、BMI和性别作为分组条件,经比较各组差异无统计学意义(P>0.05)。结论临床应用胫骨高位截骨TomoFix锁定钢板固定术对膝关节内翻性骨关节炎治疗效果较好,可有效治愈患者膝关节内翻性骨关节炎。
Objective To study the effect of TomoFix locking plate fixation on osteoarthritis of patients with knee osteoarthritis and its clinical effect.Methods A total of 40 patients with knee osteoarthritis were selected,and the function status of the patients before and after operation was evaluated using 2000 IKDC.After operation,Lysholm score,2000 IKDC score,Cincinnati score,SF-36 assessment and Tegner,Lysholm exercise level rating of patients with osteoarthritis of the tibial high osteotomy TomoFix locking plate fixation grouped according to BMI,age,gender were analyzed.The 2000 IKDC score was used to evaluate the patient's pain index and the clinical improvement of the knee varus osteoarthritis by high tibial osteotomy TomoFix locking plate fixation.Results The postoperative 2000 IKDC score was significantly higher than that of before surgery,with an improvement rate of 55.09%;the postoperative pain was significantly improved(5~10),and all patients had acceptable pain at 1 month after surgery(8~11);SF-36 evaluation postoperative physiological function score was 80.04 points,which was significantly higher than that of before surgery by 60.11 points;SF-36 psychological function postoperative score was 68.47 points,which was significantly higher than that of before surgery by 34.75 points;Tegner,Lysholm rating since level 3 was increased to level 6 before operation;age,BMI,and gender were used as the grouping conditions,there was no significant difference between the groups after comparison(P>0.05).Conclusions The clinical application of high tibial osteotomy TomoFix locking plate fixation for knee varus osteoarthritis is effective,and it could effectively cure patients with varus osteoarthritis of knee.Tibial osteotomy with TomoFix locking plate fixation has a significant improvement effect on knee osteoarthritis.
