摘要
目的比较经腹腹腔镜与开放肾盂成形术治疗肾盂输尿管连接部梗阻(UPJO)的效果。方法收治的单侧肾盂输尿管连接部梗阻患儿74例,其中经腹腹腔镜肾盂成形术34例,开放肾盂成形术40例。比较2组患者手术时间、术中出血量、术后住院时间、术后留置尿管时间、术后留置引流管时间、术后恢复饮食时间、术中并发症、手术成功率等的差异性。结果2组患者手术时间、术中出血量、术后住院时间、术后留置尿管时间、术后留置引流管时间、术后恢复饮食时间差异有统计学意义(P<0.05),2组术中并发症、手术成功率差异无统计学意义(P>0.05)。结论经腹腹腔镜肾盂成形术与开放肾盂成形术治疗肾盂输尿管连接部梗阻均安全可行,经腹腹腔镜肾盂成形术的手术时间短、术中出血少、术后恢复快,更值得临床推广。
Objective To compare the efficacy of transabdominal laparoscopy and open pyeloplasty in the treatment of ureteropelvic junction obstruction(UPJO).Methods 74 children with unilateral UPJO were treated,including 34 cases of transabdominal laparoscopic pyeloplasty and 40 cases of open pyeloplasty.The differences of the operation time,intraoperative bleeding,postoperative hospital stay,postoperative indwelling catheter time,postoperative indwelling drainage catheter time,postoperative recovery time,intraoperative complications and operative successful rate between the two groups were compared.Results The two groups had statistically significant differences in operation time,intraoperative bleeding,postoperative hospital stay,postoperative indwelling catheter time,postoperative indwelling drainage catheter time,and postoperative recovery time for eating and drinking(P<0.05).There was no significant difference in intraoperative complications and operative successful rate between the two groups(P>0.05).Conclusion The transabdominal laparoscopy and open pyeloplasty are safe and feasible for the treatment of ureteropelvic junction obstruction.The transabdominal laparoscopic pyeloplasty has the advantage of short operation time,less intraoperative bleeding,and quick postoperative recovery,which is more worthy of clinical promotion.
