照顾者引导式康复训练对脑卒中患者日常生活能力的影响

摘要 目的观察以照顾者为主导的引导式康复训练在提高脑卒中患者日常活动能力的应用效果。方法选取2019年11月至2020年7月在广西壮族自治区江滨医院康复科住院的脑卒中患者42例作为研究对象,42例患者按照随机数字表法随机分为对照组和干预组,各21例。对照组采用日常的康复训练,干预组在常规康复锻炼的基础上给予引导式教育训练。两组均每周定期随访1次,于干预前、干预后6和12周采用肢体运动功能评估表(FMA)、日常活动能力Barthal指数(BI)评估两组患者干预前后的运动功能及日常生活活动能力(activities of daily living,ADL)。结果两组患者FMA得分随时间呈上升趋势(F时间=66.572,P时间<0.001),分组与时间有交互效应(F交互=9.762,P交互=0.002),干预后6周两组比较差异无统计学意义(P=0.081),干预后12周两组比较差异有统计学意义(P=0.038);两组患者ADL得分随时间呈上升趋势(F时间=57.813,P时间<0.001),分组与时间有交互效应(F交互=13.817,P交互<0.001),干预后6周两组比较差异无统计学意义(P=0.136),干预后12周两组比较差异有统计学意义(P=0.022)。结论照顾者引导式康复训练可以提高患者的生活自理能力和康复效果,降低脑卒中患者的并发症,改善生活质量。 To observe the application effect of caregiver guided rehabilitation training in improving the daily activities of stroke patients.Methods Forty-two stroke patients hospitalized in our department were selected as research objects from November 2019 to July 2020.The 42 patients were randomly divided into a control group and an intervention group according to the random number table method,with 21 cases in each group.The control group received daily rehabilitation training,and the intervention group received caregiver guided rehabilitation training on the basis of routine rehabilitation exercises.Both groups were followed up regularly once a week.Before the intervention,6 and 12 weeks after the intervention,the simplified Fugl-Meyer motor function assessment form(FMA)and the daily activity ability Barthal index(BI)were used to evaluate the motor function showed an upward trend with time.Results The FMA scores of patients in the two groups of patients before and after intervention was statistically significant(F=66.572,P<0.001).There was an interaction effect between grouping and time(F=9.762,P=0.002),and there was no intervention in the two groups for 6 weeks(P=0.081).After 12 weeks of intervention,the FMA scores of the two groups were statistical significance(P=0.038).The ADL scores of patients in the two groups showed an upward trend with time(F=57.813,P<0.001),there was an interaction effect between grouping and time(F=13.817,P<0.001),there is no statistical significance in the two groups after 6 weeks of intervention(P=0.136),and there was statistical significance in the two groups after 12 weeks of intervention(P=0.022).Conclusion Guided caregiver rehabilitation training can improve the patient's self-care ability and rehabilitation effect,reduce the complications of stroke patients,and improve the quality of life.
