摘要
目的探究X线检查在新生儿肺透明膜病(hyaline memb-rane disease,HMD)中的诊断价值。方法选取2018年1月至2019年12月本院收治的60例HMD患儿作为研究组,另选取同期本院收治的60例吸入性肺炎患儿作为对照组,两组均采用X线诊断。比较两组X线征象情况,分析研究组X线分级情况及X线检查在诊断HMD中的应用价值。结果经X线检查,60例HMD患儿病理分级:Ⅰ级10例,Ⅱ级14例,Ⅲ级11例,Ⅳ级25例;研究组毛玻璃影检出率为63.33%,小颗粒网状影检出率为55.00%,肺纹理增粗检出率为56.67%、支气管充气征检出率为58.33%,白肺等检出率为46.67%,均高于对照组的36.67%、33.33%、30.00%、25.00%、0.00%,差异有统计学意义(P<0.05);毛玻璃影、小颗粒网状影、肺纹理增粗、支气管充气征、白肺在HMD诊断中特异度及阳性预测值比较差异有统计学意义(P<0.05)。结论采用X线检查在HMD诊断中应用价值较高,可检出不同的X线征象,可为临床疾病诊断提供参考依据。
Objective To explore the diagnostic value of X-ray examination in neonatal hyaline membrane disease(HMD). Methods 60 children with HMD admitted to our hospital from January 2018 to December 2019 were selected as the research group, and 60 children with aspiration pneumonia admitted to the our hospital during the same period were selected as the control group, both groups were diagnosed by X-ray. Compared the X-ray signs between the two groups, analyzed the X-ray classification of the study group and the application value of X-ray examination in the diagnosis of HMD. Results After X-ray examination, the pathological grades of 60 children with HMD were as follows: grade I 10 cases, grade II14 cases, grade Ⅲ 11 cases, and grade IV 25 cases. In the study group, the detection rate of hair glass shadow was 63.33%, the detection rate of small particle reticular shadow was 55.00%, the detection rate of lung texture thickening was 56.67%, the detection rate of bronchial inflation sign was 58.33%, and the detection rate of white lung was 46.67%, which were higher than 36.67%, 33.33%, 30.00%, 25.00% and 0.00% in the control group, the difference was statistically significant(P<0.05). There was significant differences in the specificity and positive predictive value of hairglass shadow, small particle reticular shadow, lung texture thickening, bronchial inflation sign and white lung in the diagnosis of HMD(P<0.05).Conclusion X-ray examination is of high value in the diagnosis of HMD, which can detect different X-ray signs and provide reference for clinical disease diagnosis.
