摘要
目的观察超声乳化白内障联合改良房角分离术治疗急性闭角型青光眼慢性期合并白内障的疗效。方法回顾性分析2016年3月至2019年11月黄山市人民医院眼科收治的28例急性闭角型青光眼慢性期合并白内障患者的临床资料,患者均采用超声乳化白内障联合改良房角分离术治疗,分析术前、术后1 d、1周、1个月、3个月、6个月患者最佳矫正视力、眼压、前房深度、前房角开放程度等情况。结果术后1 d、术后1周及术后1、3、6个月患者的最佳矫正视力均高于术前[0.50(0.40,0.60)、0.50(0.40,0.60)、0.48(0.38,0.60)、0.46(0.40,0.60)、0.45(0.35,0.55)比0.15(0.10,0.23)](P<0.001),眼压均低于术前[17.39(12.83,17.94)mmHg(1 mmHg=0.133 kPa)、17.82(13.58,18.79)mmHg、17.40(13.26,18.44)mmHg、16.79(12.87,17.74)mmHg、16.79(13.26,17.60)mmHg比31.87(27.99,33.04)mmHg](P<0.001),前房深度均大于术前[3.85(3.68,3.94)mm、3.50(3.32,3.61)mm、3.29(3.19,3.47)mm、3.20(3.12,3.31)mm、3.15(3.05,3.22)mm比1.53(1.48,1.65)mm](P<0.001)。术后1 d、术后1周及术后1、3、6个月患者的房角开放率均大于术前[100.00%(28/28)、100.00%(28/28)、96.43%(27/28)、92.86%(26/28)、92.86%(26/28)比0](P<0.001)。结论超声乳化白内障联合改良房角分离术治疗急性闭角型青光眼慢性期合并白内障疗效肯定,患者视力明显改善,眼压得到较好的控制。
Objective To observe the effect of phacoemulsification combined with modified goniosynechialysis in the treatment of acute angle closure glaucoma complicated with cataract in chronic stage.Methods The clinical data of 28 patients with acute angle closure glaucoma complicated with cataract in the chronic stage admitted to the Department of Ophthalmology of Huangshan People′s Hospital from Mar.2016 to Nov.2019 were retrospectively analyzed.All patients were treated with phacoemulsification combined with modified goniosynechialysis.The best corrected visual acuity,intrao-cular pressure,depth of anterior chamber and the angle opening rate were analyzed before operation,1 day,1 week,1 month,3 months and 6 months after operation.Results The best corrected visual acuity of the patients at 1 d,1 week and 1 month,3 months and 6 months after operation were all higher than that before operation[0.50(0.40,0.60),0.50(0.40,0.60),0.48(0.38,0.60),0.46(0.40,0.60),0.45(0.35,0.55)vs 0.15(0.10,0.23)](P<0.001),the intraocular pressures were lower than that before operation[17.39(12.83,17.94)mmHg(1 mmHg=0.133 kPa),17.82(13.58,18.79)mmHg,17.40(13.26,18.44)mmHg,16.79(12.87,17.74)mmHg,16.79(13.26,17.60)mmHg vs 31.87(27.99,33.04)mmHg](P<0.001),depths of anterior chamber were all greater than that before operation[3.85(3.68,3.94)mm,3.50(3.32,3.61)mm,3.29(3.19,3.47)mm,3.20(3.12,3.31)mm,3.15(3.05,3.22)mm vs 1.53(1.48,1.65)mm](P<0.001).The angle opening rate at 1 d,1 week and 1 month,3 months and 6 months after surgery were all higher than that before surgery[100.00%(28/28),100.00%(28/28),96.43%(27/28),92.86%(26/28),92.86%(26/28)vs 0](P<0.001).Conclusion Phacoemulsification combined with improved angle separation in the treatment of acute angle closure glaucoma complicated with cataract in chronic stage has a positive effect,which can significantly improve the patient′s visual acuity,and control the intraocular pressure well.
