摘要
目的探讨奥曲肽联合胃镜治疗消化性溃疡合并上消化道出血的临床效果。方法 24例消化性溃疡伴上消化道出血患者,随机分为对照组与观察组,每组12例。对照组进行胃镜下注射肾上腺素,观察组患者在对照组内镜治疗的基础上应用奥曲肽治疗。比较两组治疗效果、治疗前与治疗3 d后空腹胃液pH。结果观察组中显效7例(58.33%),有效4例(33.33%),无效1例(8.33%),总有效率为91.67%,对照组显效3例(25.00%), 3例(25.00%),无效6例(50.00%),总有效率为50.00%。观察组治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组患者空腹胃液pH(6.64±0.72)高于对照组的(5.31±0.58),差异具有统计学意义(P<0.05)。结论奥曲肽联合胃镜治疗消化性溃疡合并上消化道出血,疗效确切,止血效果好,值得临床推广。
Objective To discuss the clinical effect of octreotide combined with gastroscopy in the treatment of peptic ulcer complicated with upper gastrointestinal hemorrhage.Methods A total of 24 cases of peptic ulcer complicated with upper gastrointestinal hemorrhage were randomly divided into control group and observation group,with 12 cases in each group.The control group received adrenaline injection under gastroscopy,and the observation group was treated with octreotide on the basis of the control group.The therapeutic effect,pH value of fasting gastric juice before treatment and 3 d after treatment were compared between the two groups.Results In the observation group,7 cases(58.33%)were markedly effective,4 cases(33.33%)were effective,1 case(8.33%)was ineffective,and the total effective rate was 91.67%;in the control group,3 cases(25.00%)were markedly effective,3 cases(25.00%)were effective,6 cases(50.00%)were ineffective,and the total effective rate was 50.00%.The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the pH value of fasting gastric juice of the observation group was(6.64±0.72),which was higher than(5.31±0.58)of the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of octreotide and gastroscopy has affirmative efficacy and good hemostatic effect on the treatment of peptic ulcer complicated with upper gastrointestinal hemorrhage,and it is worthy of clinical promotion.
