摘要
血药浓度、抗菌谱、最低抑菌浓度都是影响抗菌药物治疗效果的重要因素。然而在耐药病原菌导致的下呼吸道感染中,静脉用药有时无法达到理想的治疗效果。目前抗菌药物雾化的使用主要集中在囊性纤维化、支气管扩张症、医院获得性/呼吸机相关性肺炎等领域,相关的临床研究表明抗菌药物雾化可能对这些患者有益。因此采用雾化的方式治疗下呼吸道耐药菌感染可能成为一个治疗趋势。抗菌药物雾化的有效性及安全性、使用人群、雾化器的选择、雾化可能导致耐药性均是临床关注的焦点,本综述从以上4点对抗菌药物雾化用药展开讨论,旨在为临床合理用药提供参考。
Plasma drug concentrations,antimicrobial spectrum,and minimum inhibitory concentration(MIC)are all important factors affecting the therapeutic effect of antimicrobial drugs.However,in the lower respiratory tract infection caused by multi-drug resistant organisms(MDRO),sometimes parenteral administration of antibiotics could not lead to the desired therapeutic effect.Currently,antimicrobial aerosols is mainly used in the fields of cystic fibrosis(CF),bronchiectasis,and nosocomial pneumonia/ventilator-associated pneumonia(VAP).Relevant clinical studies have revealed that antimicrobial aerosols may be beneficial to these people.Therefore,the use of aerosol delivery in the treatment of lower respiratory tract infection caused by MDRO may become a new trend.The efficacy and safety,rational indication,right nebulizers,and antimicrobial resistance are all the focuses of clinical attention.This review article intends to discuss the aerosol delivery of antimicrobial drugs from the above four points,and to provide preferences for clinical rational drug use.
