摘要
目的探讨围术期一体化保温方案结合体位护理对老年髋关节置换术患者术中凝血功能、应激反应及不良事件发生率的影响。方法将2018年2月至2020年5月接受髋关节置换术的86例老年患者采用随机数字表法分为对照组与观察组,各43例。对照组接受常规手术室护理,观察组在对照组基础上实施围术期一体化保温方案结合体位护理,比较两组患者的治疗效果。结果术后,观察组的PT、TT短于对照组,FIB、D-D水平低于对照组(P<0.05);手术30 min、手术结束时,观察组的E、NE水平低于对照组(P<0.05);观察组的不良事件总发生率低于对照组(P<0.05)。结论围术期一体化保温方案结合体位护理可维持老年髋关节置换术患者术中凝血功能稳定,减轻应激反应,减少术中不良事件发生率,值得推广。
Objective To explore the effects of integrated heat preservation scheme during perioperative period combined with posture care on the coagulation function,stress reaction and incidence of adverse events in elderly patients undergoing hip replacement.Methods A total of 86 elderly patients who underwent hip replacement from February 2018 to May 2020 were divided into control group and observation group by random number table method,with 43 cases in each group.The control group received routine operating room nursing,and the observation group received integrated heat preservation scheme during perioperative period combined with posture care on the basis of the control group.The treatment effects were compared between the two groups.Results After operation,PT and TT in the observation group were shorter than those in the control group,FIB and D-D levels were lower than those in the control group(P<0.05).The levels of E and NE in the observation group were lower than those in the control group at 30 min of operation and the end of operation(P<0.05).The total incidence of adverse events in the observation group was lower than that in the control group(P<0.05).Conclusion Integrated heat preservation scheme during perioperative period combined with posture care can maintain the stability of coagulation function,reduce stress response and the incidence of intraoperative adverse events in elderly patients undergoing hip replacement,which is worthy of promotion.
