摘要
目的分析主动脉夹层术后患者出院准备度情况和相关影响因素。方法选取2017年3月至2020年3月华中科技大学同济医学院附属同济医院主动脉夹层术后恢复良好准备出院的78例患者为研究对象。采用自制调查问卷收集患者性别、年龄、文化程度、手术方式、起病状态、住院时间、住院费用结算方式、自我护理能力、术后并发症和术前合并症信息,分析主动脉夹层术后患者出院准备度情况的影响因素。结果年龄、文化程度、手术方式、住院时间、自我护理能力和术后并发症是影响主动脉夹层术后患者出院准备度的单因素(P值均<0.05)。Logistic回归分析显示,年龄≥30岁、文化程度初中及以下、开放手术、住院时间<14 d、中等水平自护能力和存在术后并发症均是主动脉夹层术后患者出院准备度的危险因素(P值均<0.05)。结论主动脉夹层术后患者出院准备度情况的影响因素与患者年龄、文化程度、手术方式、住院时间、自我护理能力和术后并发症有关,医护人员应对患者具体情况进行针对性的健康宣教,以改善患者的出院准备度情况。
Objective To analyze the readiness for discharge of patients after surgery for aortic dissection and the related factors.Methods A total of 78 patients who recovered well after surgery for aortic dissection and were ready to be discharged from Tongji Hospital,Tongji Medical College of Huazhong University of Science&Technology from March 2017 to March 2020 were selected.A self-made questionnaire was used to collect patients information,including gender,age,educational level,operation method,onset status,length of hospital stay,settlement method of hospital costs,self-care ability,postoperative complications and preoperative comorbidities.The factors influencing readiness for discharge of patients after surgery for aortic dissection were analyzed.Results Age,educational level,operation method,length of hospital stay,self-care ability and postoperative complications were the single factors that affected the readiness for discharge of patients after surgery for aortic dissection(all P<0.05).Logistic regression analysis showed that age≥30 years,educational level of junior high school or below,open surgery,hospital stay<14 days,moderate self-care ability and postoperative complications were risk factors for readiness for discharge of patients after surgery for aortic dissection(all P<0.05).Conclusion The factors influencing patients readiness for discharge after surgery for aortic dissection are related to the patients age,educational level,operation method,length of hospital stay,self-care ability and postoperative complications.Medical staff should carry out targeted health education according to the specific conditions of the patients to improve their readiness for discharge.
