不同类型髌股关节撞击综合征的影像学特征及分析

摘要 目的:探讨2类髌股关节撞击综合征的影像学征象,以期为临床提供指导。方法:回顾性分析2017年1月至2020年10月经影像检查确诊的髌股关节撞击综合征患者83例,共92个患病关节,其中髌股外侧性撞击57个、髌股滑脱性撞击35个,对2类髌股关节撞击综合征的年龄及性别分布特征、影像学征象、相关解剖结构特征进行分析。结果:髌股外侧性撞击好发于19-27岁女性,滑车形态以Dejour C型为主,韧带损伤占比12/57,以髌外侧韧带的Ⅰ-Ⅱ级损伤为主,髌骨倾斜占比41/50,骨挫伤占比32/57,损伤常分布于髌骨外侧关节面软骨或皮质下骨及股骨外侧滑车关节面软骨或皮质下骨,以局灶性损伤为主。髌股滑脱性撞击好发于10-18岁女性,滑车形态以Dejour B型为主,韧带损伤占比26/35,以髌内侧韧带的Ⅱ-Ⅲ级损伤为主,髌骨脱位伴倾斜占比25/33,骨挫伤占比35/35,以髌骨内侧关节面软骨或皮质下骨及股骨外侧髁外缘皮质下骨损伤为主,损伤范围较大、程度较重。2类髌股关节撞击综合征的以上改变均具有统计学差异(P<0.05),在软骨损伤、其他结构损伤、髌骨形态等方面的改变无统计学差异(P>0.05)。结论:2类髌股关节撞击综合征的影像学特征各具特点,完整系统地分析其影像学征象有助于提高早期诊断的准确性。 Objective To explore the imaging signs of different types of patellar tendon lateral femoral condyle friction syndromes to facilitate their clinical diagnoses and treatment.Methods A retrospective analysis of 83 patients with patellar tendon lateral femoral condyle friction syndrome diagnosed by imaging from January 2017 to October 2020,with a total of 92 affected joints,including 57 cases of lateral patellofemoral impingement and 35 cases of slipped patellofemoral impingement,was performed to analyze the age and gender distribution characteristics,imaging signs,and relevant anatomical features of the 2 types of patellar tendon lateral femoral condyle friction syndromes.Results Lateral patellofemoral impingement occurred frequently in females aged 19 to 27 years and mainly had Dejour C type of trochlea,including ligament injuries with a proportion of 12/57 involving largely in gradeⅠ-Ⅱinjuries to the lateral patellar ligament,patellar tilts with a proportion of41/50 and bone contusions with a proportion of 32/57,which was often distributed to the cartilage or subcortical bone of the lateral patellar articular surface and those of the lateral femoral trochlea articular surface,with a predominance of focal injuries.Slipped patellofemoral impingement happened often in females aged 10 to 18 years and mainly had Dejour B type of trochlea,including ligament injuries with a proportion of 26/35 involving largely in gradeⅡ-Ⅲinjuries to the medial patellofemoral ligament,combined patellar dislocation and tilts with a proportion of 25/33 and bone contusions with a proportion of 35/35,which was often distributed to the cartilage or subcortical bone of the medial articular surface of the patella and those of the lateral femoral condyle,with a predominance of extensive and severe injuries.The 2 types of patellar tendon lateral femoral condyle friction syndromes were statistically different in terms of the above changes(P<0.05),while not in terms of cartilage damage,other structural damage and patellar morphology and et
