摘要
目的探讨多期动态增强CT术前预测进展期胃癌是否发生神经侵犯的诊断价值。方法回顾性收集本院48例进展期胃癌增强CT影像资料,测量肿瘤各期相CT值的最大值、最小值和平均值以及脾脏静脉期CT值(Value_(spl)),计算出增强各期相平均值与平扫CT值的差(ΔA、ΔP、ΔD),肿瘤静脉期和脾脏静脉期差(ΔT-S),对比增强比(CER_(A),CER_(P),CER_(D),CER_(T-S))和肿瘤与脾脏比(TSR),然后使用非参数Mann-Whitney u检验和接受者操作特性曲线(ROC曲线)对所有25个参数进行分析。结果有10组参数在预测神经是否发生神经侵犯中有统计学意义,ROC曲线下面积(AUC)都大于0.700,特别是ΔT-S的AUC达到了0.772。结论增强CT对术前预测胃癌是否发生神经侵犯有一定帮助,尤其是经静脉期脾脏CT值标化后的参数。
Objective To determine whether multiphasic dynamic enhanced CT can preoperatively predict perineural invasion in advanced gastric cancer.Methods A retrospective collection of enhanced CT imaging data of 48 cases of advanced gastric cancer in our hospital,the maximum,minimum,and average CT values of different phases of the tumors and the CT values of the spleen in vein phases(Value_(spl))were measured,calculate the difference between the average value of enhanced phases and the value of nonenhanced phases(ΔA,ΔP,ΔD),the difference between tumor and spleen in venous phase(ΔT-S),contrast enhancement ratio(CER_(A),CER_(P),CER_(D),CER_(T-S))and tumor-to-spleen ratio(TSR).Then all 25 parameters are analyzed by using nonparametric Mann-Whitney u test and receiver operating characteristic curve(ROC curve).Results There were 10 groups of parameters with statistical significance in predicting whether perineural invasion occurred,the area under the ROC curve(AUC)was greater than 0.700,especially the AUC ofΔT-S reached 0.772.Conclusion Enhanced CT is helpful to predict whether perineural invasion occurs in advanced gastric cancer before operation,especially the parameters after standardization of CT value of spleen in venous phase.
