摘要
目的分析老年退变性腰椎管狭窄症(DLSS)应用神经根精准减压术与传统椎板减压术的临床疗效对比。方法选择2018年7月~2020年11月收治DLSS 66例老年患者,按随机数字表法分为两组,对照组33例采用传统椎板减压术,研究组33例给予经皮全脊柱内镜下精准减压术,对比两组临床疗效。结果研究组患者治疗后VAS评分和ODI指数分别为(2.29±0.86)分、(5.77±2.26)分,均比对照组优(P<0.05);研究组患者手术时间(64.15±13.27)min、出血量(24.16±8.22)ml和住院时间(5.13±1.18)d等均短于对照组(P<0.05);研究组患者感染、神经损伤等并发症发生率9.09%,低于对照组33.33%(P<0.05)。结论经皮全脊柱内镜下精准减压术治疗DLSS老年患者,可缩短患者手术时间及住院时间,能减少其术中出血量及术后并发症发生。
Objective To analyze the clinical effect of precise decompression of nerve roots and traditional laminar decompression in elderly degenerative lumbar spinal stenosis.Methods From July 2018 to November 2020,66 elderly patients with DLSS were selected and divided into two groups according to the random number table method.The control group was treated with traditional laminectomy and 33 patients in the study group were treated with percutaneous precise decompression surgery under spine endoscopy,then compared the clinical efficacy of the two groups.Results After treatment,the VAS score and ODI index of the patients in the study group were(2.29±0.86)points and(5.77±2.26)points,respectively,which were better than those in the control group(P<0.05);the operation time of the patients in the study group was(64.15±13.27)min,the Blood loss(24.16±8.22)ml and hospitalization time(5.13±1.18)d,which were shorter than those in the control group(P<0.05);the incidence of complications such as infection and nerve damage in the study group was 9.09%,which was lower than that in the control group 33.33%(P<0.05).Conclusion Percutaneous total spine endoscopic precision decompression for the treatment of elderly patients with DLSS can shorten the operation time and hospital stay,and reduce the amount of intraoperative blood loss and postoperative complications.
