氧化苦参碱对心肌缺血-再灌注损伤保护机制的研究进展 被引量:2

摘要 心肌缺血/再灌注(ischemia reperfusion,I/R)损伤是指心脏冠状动脉内血液供应不足后,通过某些治疗手段恢复冠状动脉内血流时所引起的心肌细胞损伤,目前尚无有效治疗方法。随着I/R损伤新靶点的不断发现,中药在改善心肌I/R损伤方面的基础研究取得了一定进展。氧化苦参碱(oxymatrine,OMT)是从苦参、广豆根等中所提取出来的生物碱,在治疗心血管疾病方面有着广阔的应用前景。大量研究证实,OMT可参与多条I/R相关信号通路,减轻心肌细胞的I/R损伤。本综述从I/R相关机制入手,重点综述了OMT通过抑制氧化应激和细胞凋亡、参与I/R相关离子通道,及PI3K/Akt/GSK3β通路、TLR4-MyD88-NF-κB信号通路等I/R相关信号通路影响,以期为今后心肌I/R的基础和临床研究提供参考。 Myocardial ischemia/reperfusion(I/R)injury refers to the injury of myocardial cells caused by the recovery of blood flow after insufficient blood supply in the coronary artery.With the discovery of new targets of I/R injury,the basic research of Traditional Chinese Medicine in improving myocardial I/R injury has made some progress.Oxymatrine(OMT)is an alkaloid extracted from Sophora flavescens and Sophora alopecuroides.It has a broad application prospect in the treatment of cardiovascular diseases.A large number of studies have confirmed that OMT can participate in multiple I/R related signaling pathways and alleviate I/R injury of cardiomyocytes.This review starts with the mechanism of I/R,focusing on the effects of OMT on I/R related signaling pathways such as inhibiting oxidative stress and apoptosis,participating in I/R related ion channels,PI3K/Akt/GSK3βpathway,TLR4-MyD88-NF-κB signaling pathway,so as to provide reference for the basic and clinical research of myocardial I/R in the future.
