摘要
目的探讨Castleman病(CD)的超声声像图表现,以提高对此病的认识和超声诊断的准确性。方法回顾性分析2016年10月至2020年10月在四川省人民医院32例CD超声声像图表现,与临床分型及病理诊断对照分析,总结其声像图表现。结果32例CD,其中局限型CD27例,透明血管型19例,浆细胞型7例,混合型1例;多中心型5例,浆细胞型3例,透明血管型1例,混合型1例。超声图像表现CD透明血管型及以透明血管型为主的混合型,大部分呈椭圆形、边界清晰欠均匀低回声病灶,可见点线状高增强,内血流信号丰富,有血管穿入病灶,超声造影可见高增强或等增强、呈周边向中心增强,内部有强回声钙化灶;CD浆细胞和以浆细胞为主混合型,部分呈椭圆形、边界清晰欠均匀低回声病灶,内血流信号丰富,超声造影可见高增强或等增强、呈周边向中心增强。结论CD的超声和超声造影图像表现有一定的特征性,对CD的诊断和治疗具有重要的价值,可提高临床诊断的准确率。
Objective To explore the ultrasonographic manifestations of Castleman disease(CD)in order to improve the understanding of this disease and the accuracy of ultrasound diagnosis.Methods The ultrasonographic manifestations of 32 patients with CD in Sichuan Provincial People′s Hospital from October 2016 to October 2020 were retrospectively analyzed and compared with clinical classification and pathological diagnosis.The ultrasonographic manifestations were summarized.Results Of 32cases of CD,there were 27 cases of localized type(19 cases of transparent vascular type,7 cases of plasma cell type and 1 case of mixed type),and 5 cases of polycentric type(3 cases of plasma cell type,1 case of transparent vascular type and 1 case of mixed type).Ultrasound images showed that in CD of transparent vascular type and mixed type dominated by transparent vascular type,most of the lesions were oval shaped,with clear boundaries and inhomogeneous hypoechos,fine dotted line like high enhancement could be seen,with rich internal blood flow signal and blood vessels penetrating the lesion.Contrast-enhanced ultrasound could be seen with high or equal enhancement,peripheral to the center enhancement,and strong echogenic calcification in the interior.In CD of plasma cells type and mixed type mainly based on plasma cells,part of the lesions showed oval shape,clear boundary and inhomogeneous hypoecho,rich internal blood flow signal.Contrast-enhanced ultrasound showed high enhancement or iso-enhancement,peripheral to the center enhancement.Conclusion The ultrasound and contrast-enhanced ultrasound images of CD have certain characteristics,which is of great value to the diagnosis and treatment of CD,and can improve the accuracy of clinical diagnosis.
