摘要
目的系统评价急性白血病患儿照顾者的真实体验及需求。方法计算机检索PubMed、Ebsco、Web of science、Medline及Cochrane Library 5个英文数据库和中国知网(CNKI)、万方及维普3个中文数据库中关于急性白血病患儿照顾者真实体验及需求的质性研究,检索时限从建库至2020年11月。采用澳大利亚JBI循证卫生保健中心质性研究质量评价标准进行质量评价后,采用Meta汇集性整合方法对纳入的文章进行整合。结果共纳入15篇文献,提炼出56个明确结果,归纳形成9个新类别,最终形成3个整合结果:(1)急性白血病患儿照顾者因支持系统较薄弱且要承担多重压力和负担而出现了多方面的担忧和复杂的心理体验。(2)儿童急性白血病是一种家庭性疾病,多维度影响到照顾者个人、家庭和社会方面。(3)急性白血病患儿照顾者因缺乏疾病知识和应对策略,渴望得到多方面的照护与支持,产生了多元化需求。结论家庭、医疗机构和社会应多关注急性白血病患儿照顾者,并为其提供心理、社会及经济支持,以促进白血病患儿及照顾者的身心健康。
Objective To systematically evaluate the real experiences and needs of caregivers of acute leukemia children.Methods 5 English databases including PubMed,Ebsco,Web of Science,Medline,Cochrane Library and 3 Chinese databases including CNKI,Wanfang and VIP were searched by computer.Qualitative studies on the real experiences and needs of caregivers of acute leukemia children was searched,and the retrieval time was inception to November 2020.The Joanna Briggs institute critical appraisal tool qualitative researches in Australia was used to evaluate quality of the studies.The results were integrated by a meta-synthesis method.Results A total of 15 studies were included,58 findings were extracted,9 new categories were generated and 3 integrated results were finally formed.Results 1:Because of the weak support system,the multiple pressures and burdens,the caregivers of children with acute leukemia had many worries and complex psychological experiences.Results 2:Acute childhood leukemia is a familial disease that affects individual caregivers,families and society in multiple dimensions.Results 3:Due to the lack of disease knowledge and coping strategies,the caregivers of children with acute leukemia longed for various care and support,and diversified demands were generated.Conclusion Families,medical institutions and society should pay more attention to the caregivers of children with acute leukemia and provide them with psychological,social and economic support to promote the physical and mental health of the children and caregivers.
