摘要
目的:比较腰硬联合麻醉与硬膜外麻醉用于无痛分娩的效果。方法:抽取2018年5月至2019年11月于太原市第八人民医院行无痛分娩的产妇125例,根据麻醉方式分为两组。观察组64例采用腰硬联合麻醉,对照组61例采用单项硬膜外麻醉。回顾性比较两组镇痛效果、产程、新生儿情况及不良反应。结果:观察组麻醉起效时间短于对照组(t=14.387,P<0.05)。两组随着麻醉时间延长(5、10、30、60 min),视觉模拟量表(VAS)评分逐渐下降,且观察组相应值低于对照组(t=26.855、17.317、14.124、13.999,P<0.05)。观察组活跃期时间和第二产程时间短于对照组(t=4.048、1.998,P<0.05)。两组产妇第三产程时间、新生儿Apgar评分比较,t=0.750、1.419,P>0.05。两组产妇运动阻滞、不良反应发生率比较,χ^(2)=3.016、0.005,P>0.05。结论:腰硬联合麻醉用于无痛分娩镇痛效果优于硬膜外麻醉,且可缩短活跃期时间和第二产程时间。
Objective To compare the effects of combined spinal-epidural anesthesia and epidural anesthesia on painless labor.Methods A total of 125 atients of painless delivery admitted to the Eighth People’s Hospital of Taiyuan from May 2018 to November 2019 were selected and divided into two groups according to anesthesia methods.Sixty-four patients in the observation group received combined spinal-epidural anesthesia,and 61 patients in the control group were treated with epidural anesthesia.Analgesic effects,labor process,neonatal conditions and adverse reactions were retrospectively compared between the two groups.Results The onset time of anesthesia in the observation group was shorter than that in the control group(t=14.387,P<0.05).The VAS scores of the two groups gradually decreased with the the prolonged anesthesia time(5,10,30,60 min),and corresponding value in the observation group was lower than those in the control group(t=26.855,17.317,14.124,13.999,P<0.05).The active period time and the time of the second stage of labor in the observation group were shorter than those in the control group(t=4.048,1.998,P<0.05).The time of the third stage of labor and Apgar score of newborns between the two groups was compared,t=0.750,1.419,P>0.05.The incidence of motor block and adverse reactions between the two groups was compared,χ^(2)=3.016,0.005,P>0.05.Conclusions The analgesic effect of combined spinal-epidural anesthesia in painless labor is better than that in epidural anesthesia,and it can shorten the time of the active phase and the second stage of labor.
