丙种球蛋白对小儿川崎病的疗效及影响因素分析 被引量:9

摘要 目的观察丙种球蛋白治疗小儿川崎病(KD)的疗效,并分析疗效的影响因素。方法回顾分析2016年5月至2020年5月住院并完成丙种球蛋白治疗的92例小儿KD患儿资料,全部患儿均完成剂量2 g/kg,7 d 1次的丙种球蛋白治疗,7 d均接受疗效评估,设计资料填写表仔细查阅患儿资料并记录研究所需基线资料,经单因素与多因素分析找出影响丙种球蛋白治疗小儿KD的疗效的因素。结果2 g/kg治疗,92例小儿KD患儿中,治疗无效15例,治疗无效率16.30%;初步比较不同疗效患儿基线资料、实验室指标和血清指标后,将比较差异有统计学意义变量纳入作为自变量行单项回归分析,后将全部资料纳入建立多元回归模型,结果显示,发热持续时间长、CRP、血沉、SF过表达和血浆白蛋白低表达均是KD患儿丙种球蛋白治疗无效的影响因素。结论发热持续时间长、CRP、血沉、SF过表达、血浆白蛋白低表达均可能提示KD患儿经丙种球蛋白治疗无效高风险,临床应重视有上述风险患儿的早期合理干预,可能对提高治疗效果有积极意义。 Objective To investigate the therapeutic effects of gamma globulin on Kawasaki disease(KD)in children and its influencing factors.Methods The clinical data about 92 children with KD who were treated by gamma globulin in our hospital from January 2018 to February 2019 were retrospectively analyzed,with a treatment course of 14 days.After treatment,the therapeutic effects were evaluated,the design data were analyzed by single factor and multiple factor analysis to investigate the factors affecting the curative effects of gamma globulin on KD.Results After 14-day treatment,of 92 patients,15 were ineffective in treatment,with the ineffective rate being 16.30%.After a preliminary comparison of baseline data,laboratory indicators and serum indicators of children with different curative effects,the variables with statistical significance were included as independent variables,and a multiple regression model was established.The results showed that long duration of fever,over expression of CPR,erythrocyte sedimentation rate,SF and low expression of plasma albumin were the influencing factors of ineffective treatment of gamma globulin in treatment of KD.Conclusion The long duration of fever,over expression of CPR,erythrocyte sedimentation rate,SF and low expression of plasma albumin may indicate the high risk of ineffective treatment of gamma globulin in children with KD,therfor,it is necessary to take early and reasonable intervention to improve the curatice effects on KD.
