摘要
目的:运用数据挖掘分析针灸治疗桡骨茎突狭窄性腱鞘炎的选穴共性和规律。方法:检索中国知网、万方数据库、维普中文期刊数据库和Pubmed数据库建库至2020年12月收录的针灸治疗桡骨茎突狭窄性腱鞘炎的临床研究文献,用Excel 2019软件建立针灸处方主穴数据库,采用数据挖掘技术中的关联规则方法,分析针灸处方经穴运用的特点和规律。结果:最终得到45篇符合纳入标准的文献,共纳入针灸处方45张。使用频次最高的穴位、经络和部位分别为阿是穴、手阳明大肠经和上肢腕关节。关联规则结果显示,置信度最高的穴位组合为手三里-列缺-阳溪,支持度最高的穴位组合是列缺-阳溪。聚类分析结果显示,存在2个有效聚类群,分别为“列缺-阳溪-合谷”“鱼际-温溜-太渊-外关-曲池-手三里”。结论:临床治疗桡骨茎突狭窄性腱鞘炎应以阿是穴为主穴,并结合具体情况配合使用列缺、阳溪、合谷等穴位进行施治。
Objective:To analyze the generality and laws of acupoint selection in acupuncture treatment of stenosing tenosynovitis of radial styloid process by data mining.Methods:We searched CNKI,Wanfang database,VIP Chinese Journal Database and PubMed database for clinical research literatures on acupuncture treatment of stenosing tenosynovitis of radial styloid process from the establishment of the database to December 2020,established the main acupoints′database of acupuncture formulas with Excel 2019 software,and analyzed the characteristics and laws of acupuncture formula acupoint application by using the association rule method of data mining technology.Results:Finally,a total of 45 literatures meeting the inclusion criteria were obtained,including 45 acupuncture formulas.The most frequently-used acupoints,meridians and parts were Ashi point,large intestine meridian of hand yang brightness and upper limb wrist joint respectively.The results of association rules showed that the combination of acupoints with the highest confidence was Shousanli(LI 10)-Lieque(LU 7)-Yangxi(LI 5),and the combination of acupoints with the highest support was Lieque(LU 7)-Yangxi(LI 5).The cluster analysis results showed that there were two effective cluster groups,which were"Lieque(LU 7)-Yangxi(LI 5)-Hegu(LI 4)",and"Yuji(LU 10)-Wenliu(LI 7)-Taiyuan(LU 9)-Waiguan(SJ 5)-Quchi(LI 11)-Shousanli(LI 10)".Conclusion:The clinical treatment of stenosing tenosynovitis of styloid process of radius should be based on Ashi point,and combined with the specific situation with the use of Lieque(LU 7),Yangxi(LI 5),Hegu(LI 4)and other acupoints.
