摘要
目的:分析右美托咪定联合氢吗啡酮在产科患者术后镇痛镇静的效果及对免疫功能的影响。方法:选取2018年3月-2020年3月在笔者所在医院需行产科手术患者154例作为研究对象,根据随机数字表法将其分为对照组和观察组,各77例。对照组术后给予右美托咪定联合舒芬太尼静脉自控镇痛,观察组术后给予右美托咪定联合氢吗啡酮静脉自控镇痛。比较两组术后不同时点VAS评分和Ramsay镇静评分及免疫功能相关指标的变化。结果:观察组术后不同时点VAS评分均低于对照组,差异有统计学意义(P<0.05)。两组术后不同时点Ramsay镇静评分比较差异无统计学意义(P>0.05)。两组术后不同时点CD4+、CD8+及NK细胞均较术前降低,差异有统计学意义(P<0.05)。两组术后不同时点CD8+比较差异无统计学意义(P>0.05)。术后12、24、48 h观察组CD4+均高于对照组,差异有统计学意义(P<0.05),术后6、12、24、48 h观察组NK细胞均高于对照组,差异有统计学意义(P<0.05)。结论:右美托咪定联合氢吗啡酮可降低产科患者术后疼痛,可改善术后免疫抑制,且不会影响患者的镇静评分,值得临床应用推广。
Objective:To analyze the effect of Dexmedetomidine combined with Hydromorphone on postoperative analgesia and sedation in obstetric patients and the effects on immune function.Method:A total of 154 patients who undergoing obstetric surgery in our hospital from March 2018 to March 2020 were selected as the research objects,and were divided into the control group and the observation group according to the random number table method,with 77 cases in each group.The control group was given Dexmedetomidine combined with Sufentanil intravenous self-controlled analgesia,and the observation group was given Dexmedetomidine combined with Hydromorphone intravenous self-controlled analgesia.The changes of VAS score,Ramsay sedation score and related indexes of immune function were compared between the two groups at different time points after surgery.Result:The VAS score of the observation group at different time after surgery were lower than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in Ramsay sedation scores between the two groups at different time points after surgery(P>0.05).The levels of CD4+,CD8+and NK cells in the two groups at different time points after surgery were significantly lower than those before surgery,the differences were statistically significant(P<0.05).There was no significant difference in CD8+between the two groups at different time after surgery(P>0.05).After 12,24 and 48 h,the CD4+of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After 6,12,24 and 48 h,the NK cells in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Dexmedetomidine combined with Hydromorphone can reduce postoperative pain in obstetric patients,improve postoperative immunosuppression,and will not affect the sedation score of patients,which is worthy of clinical application and promotion.
