Hypermetabolic response induced by severe burn and strategies for management

摘要 严重烧伤可引起强烈且持久的高代谢反应,是烧伤应激反应的标志之一,与烧伤兴奋神经-内分泌系统,诱导儿茶酚胺、糖皮质激素和胰高血糖素等应激激素释放,导致急性高代谢反应、高分解代谢状态有关。本文回顾了严重烧伤后高代谢反应的主要病理生理改变,包括白色脂肪组织(WAT)棕化在其中的重要作用,以及烧伤后高代谢反应对机体蛋白质、脂肪及糖代谢的影响。本文亦对调控烧伤后高代谢反应的药物及其作用机制,以及高代谢反应的综合治疗策略进行了全面阐述,并对调控烧伤后高代谢率的新技术手段及其对提高严重烧伤救治成功率的意义进行了展望。 Severe burns induced a strong and long-lasting hypermetabolic response,which is a hallmark of the stress response after burn injury.It is associated with the release of catecholamines,glucocorticoids and glucagon in plasma after the neuroendocrine system triggered by burn injury,resulting in acute hypermetabolic response and subsequent catabolic states.In this article,the main pathophysiological variation during hypermetabolic response after severe burn were reviewed including the crucial effect of the browning of white adipose tissue(WAT)involved,as well as the effects of hypermetabolic response after severe burn on the body’s protein,fat,glucose metabolism.Besides,the drugs modulating the hypermetabolic response and the associated mechanisms,and the other strategies with comprehensive treatment were also thoroughly discussed in this article.The new technological method and its significance on increasing the success rate of severe burn treatment were finally envisioned.
