摘要
目的收集良性前列腺增生(BPH)患者行经尿道前列腺电切术(TURP)围手术期相关临床信息,构建BPH患者行TURP治疗是否有效的列线图预测模型。方法选取2016年5月至2018年12月贵州省人民医院泌尿外科收治的226例行TURP治疗的BPH患者作为研究对象,进行回顾性分析。收集其临床数据,使用受试者操作特征(ROC)曲线计算相关临床数据的约登指数作为阈值,然后根据阈值转换数据后构建BPH患者行TURP治疗是否有效的预测模型并验证其辨识能力及准确度。结果选取前列腺体积是否大于42mL、移行带体积(TZV)是否大于23mL、移行带指数(TZI)是否大于0.54、膀胱出口梗阻指数(BOOI)是否大于44及残余尿(PVR)构建预测模型,依据以上临床指标建立列线图预测模型后,预测模型ROC曲线下面积为0.711(95%CI为0.555~0.828)。结论前列腺体积是否大于42mL、TZV是否大于23mL、TZI是否大于0.54、BOOI是否大于44及PVR构建预测模型可以很好地预测BPH患者行TURP治疗是否有效的概率。
Objective To collect the perioperative clinical information of patients with benign prostatic hyperplasia(BPH) undergoing transurethral resection of the prostate(TURP), and construct a nomogram model to predict the efficacy of TURP therapy. Methods Clinical data of patients undergoing TURP due to BPH in the Department of Urology of Guizhou Provincial People′s Hospital from May 2016 to December 2018 were collected. The ROC curve was used to calculate the Jorden index of relevant clinical data as the threshold value, and then a prediction model for the effectiveness of TURP treatment for BPH patients was constructed based on the converted threshold data and its identification ability and accuracy were verified. Results The prediction model was constructed by selecting whether the prostate volume was greater than 42 mL, the transitional zone volume(TZV) was greater than 23 mL, the transitional zone index(TZI) was greater than 0.54, the bladder outlet obstruction index(BOOI) was greater than 44 and postvoid residual urine(PVR). After the histogram prediction model was established according to the above clinical indicators, The area under the ROC curve of the prediction model was 0.711(95%CI 0.555~0.828). Conclusions Whether the prostate volume is greater than 42 mL, TZV is greater than 23 mL, TZI is greater than 0.54, BOOI is greater than 44, and PVR model can well predict the probability of effective TURP treatment in BPH patients.
