摘要
目的比较低温等离子射频消融术与喉钳钳取术治疗喉部良性肿瘤的疗效。方法前瞻性抽取2016年5月至2019年5月驻马店市中心医院收治的喉部良性肿瘤患者204例,将其随机分为对照组和观察组,每组102例。对照组给予喉钳钳取术治疗,观察组给予低温等离子射频消融术治疗。比较两组患者术后6个月的临床总有效率、术后1周的咽部异物感和喉部的疼痛程度、围术期相关指标及术后并发症发生情况,并对两组患者进行为期1年的随访观察,比较两组术后1年的复发率。结果术后6个月,观察组临床总有效率(98.04%,100/102)与对照组(96.08%,98/102)比较,差异未见统计学意义(P>0.05)。术后1周,两组患者的咽部异物感视觉模拟评分法(VAS)评分和喉部嗓音VAS评分与同组术前比较均下降(P<0.05),但两组间比较差异未见统计学意义(P>0.05)。观察组手术时间、切口愈合时间及住院时间均短于对照组(P均<0.05),术中出血量少于对照组(P<0.05)。两组术后活动性出血和声带粘连发生率比较差异未见统计学意义(P>0.05)。观察组术后1年的复发率(1.96%,2/102)低于对照组(8.82%,9/102),P<0.05。结论与喉钳钳取术比较,低温等离子射频消融术治疗喉部良性肿瘤能达到相同的治疗效果,减轻患者咽喉部疼痛程度,且低温等离子射频消融术的手术时间、切口愈合时间更短,术中出血量更少,住院时间更短,术后复发率更低。
Objective To compare the efficacy of low-temperature plasma radiofrequency ablation and laryngeal forceps in the treatment of benign laryngeal tumors.Methods A total of 204 patients with laryngeal benign tumor treated in Zhumadian Central Hospital from May 2016 to May 2019 were prospectively selected.All the patients were randomly divided into control group and observation group,with 102 cases in each group.The control group was treated by laryngeal forceps extraction,and the observation group was treated by low-temperature plasma radiofrequency ablation.The 6-month postoperative clinical total effective rate,foreign body sensation in pharynx and laryngeal pain at 1 week after operation,perioperative related indexes and postoperative complications were compared between the two groups.The two groups were followed up for 1 year.And the recurrence rate of the two groups 1 year after operation was compared between the two groups.Results The clinical total effective rate of the observation group(98.04%,100/102)was not significantly different from that of the control group(96.08%,98/102)6 months after surgery(P>0.05).At 1 week after surgery,the visual analogue scale(VAS)score of pharyngeal foreign body sensation,and VAS score of larynx voice of the two groups were decreased,compared with those before operation(P<0.05),but there were no statistically significant differences between the two groups(P>0.05).The surgical time,wound healing time and hospital stay in observation group were significantly shorter than those in control group(P<0.05),and the intraoperative blood loss was significantly less than that in control group(P<0.05).There was no significant difference in the incidences of postoperative complications of active bleeding and vocal cord adhesion between the two groups(P>0.05).The 1-year postoperative recurrence rate in observation group(1.96%,2/102)was significantly lower than that in control group(8.82%,9/102),P<0.05.Conclusions Compared with laryngeal forceps,low-temperature plasma radiofrequency ablation i
