两种术式对股骨转子间骨折患者术后骨密度及下肢功能的影响分析 被引量:4

摘要 目的:分析动力髋螺钉(DHS)和股骨近端抗旋髓内钉(PFNA)对股骨转子间骨折患者术后骨密度及下肢功能的影响。方法:选取91例股骨转子间骨折患者为研究对象,根据手术方式不同分为DHS组(n=43)和PFNA组(n=48);比较两组手术相关指标、术后并发症、骨密度及下肢功能。结果:PFNA组患者手术时间、切口长度短于DHS组(P<0.05),术中出血量和住院时间少于DHS组(P<0.05);两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05);术后3个月,两组患者外侧粗隆下骨密度均降低(P<0.05),且PFNA组大于DHS组(P<0.05);两组患者Harris髋关节功能评分比较,差异无统计学意义(P>0.05);术后6个月,PFNA组患者Harris髋关节功能评分高于DHS组(P<0.05)。结论:DHS和PFNA均能有效治疗股骨转子间骨折,但PFNA手术切口小,术后恢复快,骨密度水平高,下肢功能恢复效果更显著,在治疗中更具优势。 Objective:To analyze the influences of dynamic hip screw(DHS)and proximal femoral nail anti-rotation(PFNA)on postoperative bone mineral density level and lower limb function in patients with intertrochanteric fractures.Methods:91 patients with intertrochanteric fractures were selected as the research objects.According to the surgical method,they were divided into DHS group(n=43)and PFNA group(n=48).The operation related indexes,postoperative complications,bone mineral density level and lower limb function were compared between the two groups.Results:The operation time and incision length of the PFNA group were shorter than those of the DHS group(P<0.05).The intraoperative blood loss and hospital stay were less than those of the DHS group(P<0.05).There was no significant difference in the total incidence of complications between PFNA group and DHS group(P>0.05).At 3 months after operation,the bone mineral density under the lateral trochanter in both groups was decreased(P<0.05),and the PFNA group was larger than the DHS group(P<0.05).There was no significant difference in Harris hip function score between the PFNA group and the DHS group at 3 months after surgery(P>0.05).The Harris hip function score of the PFNA group was significantly higher than that of the DHS group at 6 months after surgery(P<0.05).Conclusion:DHS and PFNA can effectively treat intertrochanteric fracture of femur,but PFNA has small incision,fast recovery after operation,high bone density,more significant lower limb function recovery effect,and has more advantages in the treatment of intertrochanteric fractures.
