摘要
目的:探究宫腔镜下冷刀治疗在宫腔粘连中的临床疗效及对子宫内膜血流的影响。方法:选取2020年1-10月80例宫腔粘连患者,随机分为对照组和观察组,每组40例。对照组采用传统宫腔镜电切分离术,观察组采用宫腔镜下冷刀治疗。比较两组疗效、月经改善情况、粘连复发率、子宫内膜血流类型和子宫内膜螺旋动脉血流参数。结果:术后1、3个月,观察组总有效率高于对照组,月经改善情况优于对照组,差异均有统计学意义(P<0.05)。术后1、2、3个月,观察组粘连复发率均低于对照组,差异均有统计学意义(P<0.05)。术前,两组子宫内膜血流类型、子宫内膜螺旋动脉血流参数比较,差异均无统计学意义(P>0.05);术后1、3个月,观察组子宫内膜血流类型、子宫内膜螺旋动脉血流参数均优于对照组,差异均有统计学意义(P<0.05)。结论:宫腔镜下冷刀治疗宫腔粘连的临床效果较好,且改善子宫内膜血流状态,在宫腔粘连患者中的应用价值较高。
Objective:To investigate and observe the clinical effect of hysteroscopic cold knife treatment in the patients with intrauterine adhesions and influence on endometrial blood flow.Method:A total of 80 patients with intrauterine adhesions from January 2020 to October 2020 were selected,and they were randomly divided into control group and observation group,40 cases in each group.The control group was treated with traditional hysteroscopic electrotomy and the observation group was treated with hysteroscopic cold knife treatment.The curative effect,menstrual improvement,adhesion recurrence rate,endometrial blood flow type and endometrial spiral artery blood flow parameters were compared between two groups.Result:1 month and 3 months after surgery,the total effective rate of observation group were higher than those of control group,and the menstruation improvement was superior than that of the control group,the differences were statistically significant(P<0.05).1 month,2 months and 3 months after surgery,the rate of adhesion recurrence of observation group were lower than those of control group,and the differences were statistically significant(P<0.05).Before surgery,there were no statistically significant differences in endometrial blood flow type and endometrial spiral artery blood flow parameters between two groups(P>0.05).The endometrial blood flow type and the endometrial spiral artery blood flow parameters of observation group were superior than those of control group,and the differences were statistically significant(P<0.05).Conclusion:The clinical effect of hysteroscopic cold knife treatment in the patients with intrauterine adhesions is better,and it can significantly improve the endometrial blood flow state,so its application value in the patients with intrauterine adhesions is higher.
