摘要
目的探讨康柏西普玻璃体腔注药联合视网膜激光光凝术治疗局灶性糖尿病性黄斑水肿(DME)的临床疗效。方法56例(84眼)局灶性DME患者,随机分为对照组(31例,44眼)和观察组(25例,40眼)。对照组行单纯玻璃体腔注射康柏西普治疗,观察组行玻璃体腔注射康柏西普联合视网膜激光光凝术治疗。比较两组患者治疗前和治疗后1、3、6个月的最佳矫正视力(BCVA)、黄斑中心凹厚度(CMT),注射药物次数,并发症发生情况。结果治疗前,两组BCVA、CMT比较,差异均无统计学意义(P>0.05);治疗后1、3、6个月,观察组BCVA、CMT均优于对照组,差异具有统计学意义(P<0.05)。对照组注射药物次数为(2.4±0.7)次,与观察组的(2.2±0.5)次比较,差异无统计学意义(P>0.05)。术后随访6个月,所有患者术后均未发生眼内炎、玻璃体积血、视网膜脱离、白内障等并发症。结论对局灶性DME玻璃体腔注射康柏西普联合黄斑局灶样激光光凝术安全有效,术后无明显并发症的发生,而且联合治疗可以更好的降低患者黄斑水肿的厚度,改善患者的视功能,但长期疗效还有待于进一步观察。
Objective To discuss the clinical efficacy of intravitreal conbercept injection combined with retinal laser photocoagulation in the treatment of focal diabetic macular edema(DME).Methods A total of 56 patients(84 involved eyes)with focal DME were randomly divided into control group(31 cases,44 cases)and observation group(25 cases,40 eyes).The control group was treated with intravitreal conbercept injection,and the observation group was treated with intravitreal conbercept injection and retinal laser photocoagulation.The best-corrected visual acuity(BCVA)and central macular thickness(CMT)before treatment and after 1,3,6 months of treatment,number of injections,and the occurrence of complications were compared between the two groups.Results Before treatment,there was no statistically significant difference in BCVA and CMT between the two groups(P>0.05).After 1,3,6 months of treatment,the BCVA and CMT of the observation group were better than those of the control group,and the difference was statistically significant(P<0.05).The number of injections of the control group was(2.4±0.7)times,which had no statistically significant difference compared with(2.2±0.5)times of the observation group(P>0.05).All patients were followed up for 6 months,and there was no complications such as endophthalmitis,vitreous hemorrhage,retinal detachment or cataract occurred.Conclusion Combination of intravitreal Conbercept injection and retinal laser photocoagulation is safe and effective on focal DME.There are no obvious complications after the operation,and the combined treatment can better reduce the thickness of macular edema and improve the patient’s visual function,but the long-term efficacy remains to be further observed.
