摘要
急性肺损伤(acute lung injury,ALI)/急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)因发病迅猛和高病死率已成为现代医学面临的重大临床问题。ALI/ARDS的发生、发展涉及多种机制,过度炎症反应和肺实质细胞凋亡被认为是引发ALI/ARDS病理损伤的主要原因,大量研究表明,内质网应激(endoplasmic reticulum stress,ERS)与上述机制密切相关。本综述强调了ERS与肺泡上皮细胞、肺泡巨噬细胞的串扰在ALI/ARDS发病中的关键作用,并对近年来抑制ERS信号的特异性抑制剂、中药单体和化学药物进行总结,以期为ERS在ALI/ARDS的临床应用中提供理论依据。
Acute lung injury( ALI)/acute respiratory distress syndrome( ARDS) has become a major clinical problem in modern medicine because of its rapid onset and high mortality. The occurrence and development of ALI/ARDS involves a variety of mechanisms,among which excessive inflammatory reaction and apoptosis of pulmonary parenchymal cells are considered as the most prominent pathophysiological change of ALI/ARDS. A large number of studies have shown that endoplasmic reticulum stress( ERS) is closely related to the above mechanisms. In this review, the author emphasizes the key role of ERS crosstalk with alveolar epithelial cells and alveolar macrophages in the pathogenesis of ALI/ARDS and summarizes the specific inhibitors, traditional Chinese medicine monomers,and chemical drugs that inhibit ERS in recent years,in order to provide theoretical basis for the clinical application of ERS in ALI/ARDS.
