摘要
目的:对备战2022年北京冬奥会自由式滑雪空中技巧优秀运动员肘关节屈肌肌腱及韧带重建术后康复训练全过程进行归纳和总结,以期为肘关节肌腱、内侧副韧带及其他韧带重建术后康复训练目标、方案及监控评估方式提供借鉴和参考。方法:采用文献资料法、专家访谈法和测试法,对肘关节屈肌肌腱、内侧副韧带重建术后的康复训练进行了研究。结果:通过7周的康复训练,患侧肘关节疼痛度与肿胀度指数降为0,两侧肘关节屈伸角度对称度达到100%,两侧手臂推拉肌力水平对称度达到75%,患肢承受总负荷达到健肢的96.2%,两侧大臂肌肉围度的对称度达到99.3%。结论:主观感受和客观数据相结合的监控评估方式可以更深入把握损伤部位的恢复情况;相邻关节耐受性肌力训练有利于促进患侧关节肿胀消除,有利于减缓疼痛;手法松解与神经肌肉促进法相结合的方式更有利于促进关节灵活度的恢复;康复训练更需在可控的训练负荷下重点恢复和重塑神经支配肌肉的能力,渐进增加训练负荷和变化训练方式来最大程度地发展患肢肘关节相邻关节周围的肌肉维度,有利于患肢肘关节周围肌肉维度的发展;下肢和躯干大强度的有氧代谢和心肺功能训练,有利于促进疏通患肢损伤部位的微循环系统和运动功能的恢复。
Objective:To provide reference for rehabilitation training objectives,program strategies and monitoring evaluation methods after reconstruction of elbow tendon,medial collateral ligament and other ligaments by summarizing the whole process of rehabilitation training for elite freestyle skiers after elbow flexor tendon and ligament reconstruction in preparation for 2022 Beijing Winter Olympic Games.Methods:By using the methods of literature review,expert interview and test,the rehabilitation training of elbow flexor tendon and medial collateral ligament reconstruction was studied.Results:After 7 weeks of rehabilitation training,the pain and swelling index of affected elbow joint decreased to 0,the symmetry of flexion and extension angle of both elbow joints reached 100%,the symmetry of push-pull muscle strength of both arms reached 75%,the total load of affected limb reached 96.2%of that of healthy limb,and the symmetry of muscle circumference of both arms reached 99.3%.Conclusions:The combination of subjective feelings and objective data can better grasp the recovery of the injured part;The tolerance training of adjacent joints is helpful to promote the elimination of swelling of affected joints and relieve pain;The combination of manipulation release and neuromuscular promotion is more conducive to the recovery of joint flexibility;Rehabilitation training should focus on the development of the ability to remodel the innervated muscles under the controllable training load,gradually increase the training load and change the training methods to maximize the development of the muscle dimension around the elbow joint of the affected limb,which is conducive to the development of the muscle dimension around the elbow joint of the affected limb;High intensity aerobic metabolism and cardiopulmonary function training of healthy limbs can promote the recovery of microcirculation system and motor function of injured limbs.
