摘要
目的探讨乳腺癌基因-1(BRCA1)、胞命运决定因子(DACH1)及S100A4在子宫内膜癌(EC)病情进展及治疗预后中的预测价值。方法选取2016年3月至2018年9月收治的127例EC患者(127份EC组织)设为EC组,选取同期于本院进行手术治疗的133例子宫内膜非典型增生患者(133份子宫内膜非典型增生组织)设为对照组。比较2组BRCA1、DACH1及S100A4水平,分析三者与EC病理特征的关系。随访2年,记录患者预后情况。绘制ROC曲线,分析不同BRCA1、DACH1及S100A4表达对EC预后的预测价值。结果 EC组BRCA1、DACH1阴性率以及S100A4阳性率显著高于对照组,差异均有统计学意义(P<0.05)。组织学分级为G1、FIGO分期为Ⅲ~Ⅳ期、有淋巴结转移的EC患者BRCA1、DACH1阴性率、S100A4阳性率更高(P<0.05)。127例EC患者术后1年生存率为82.67%(105/127)。BRCA1及DACH1阴性表达、S100A4阳性表达死亡率显著高于BRCA1及DACH1阳性表达、S100A4阴性表达死亡率,差异均有统计学意义(P<0.05)。ROC曲线显示:联合BRCA1、DACH1、S100A4检测灵敏度、特异度、AUC分别为89.65%、90.73%、0.962,各指标检测灵敏度、特异度、AUC以三者联合检测最大(P<0.05)。结论 BRCA1、DACH1及S100A4与EC发生、发展、预后关系密切,临床可通过联合三项指标检测评估EC患者病情,预测预后。
Objective To explore the value of breast cancer gene⁃1(BRCA1),cell fate determination factor Dachshund(DACH1)and S100A4 in predicting the progression and prognosis of endometrial cancer(EC).Methods The 127 EC patients admitted to this hospital from March 2016 to September 2018 were selected as the EC group(127 EC tissues),and the 133 patients with endometrial atypical hyperplasia treated with surgery during the same period were as the control group(133 atypical hyperplasia tissues of the endometrium).The levels of BRCA1,DACH1 and S100A4 in the two groups were compared,and the relationship between the three levels and the pathological characteristics of EC was analyzed.The patients were followed up for 2 years,and the prognostic outcome of the patients was recorded.The ROC curve was drawn to analyze the value of BRCA1,DACH1 and S100A4 expressions in predicting the prognosis of patients with EC.Results The negative rates of BRCA1 and DACH1 and the positive rate of S100A4 in the EC group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The negative rates of BRCA1 and DACH1 and the positive rate of S100A4 in patients with histological grade at G1,FIGO stage atⅢ~Ⅳand lymph node metastasis are higher than those of the positive expression rates of BRCA1,DACH1 and negative expression rate of S100A4,the difference was statistically significant (P<0.05). The 1 ⁃ year survival rate of 127 EC patients was 82.67%(105/127). The mortality of patients with negative expressions of BRCA1 and DACH1 and positive expression of S100A4 was higher than that of BRCA1 and DACH1 positive expressions and S100A4 negative expression, and the difference was statistically significant (P<0.05). The ROC curve showed that the sensitivity,specificity,and AUC of the combined detection of BRCA1,DACH1,and S100A4 were 89.65%,90.73%,and 0.962,respectively.
