不同剂量肺表面活性物质对预防低出生体重儿慢性肺病的临床效果 被引量:1

Clinical Effect of Different Doses of Pulmonary Surfactant on Prevention of Chronic Lung Disease in Low Birth Weight Infants

摘要 目的探讨气管内滴入不同剂量肺表面活性物质对预防低出生体重儿慢性肺病(CLD)的临床效果。方法选取2019年1月至2020年12月广东医科大学顺德妇女儿童医院新生儿科收治的早产儿120例作为研究对象,随机分为试验组与对照组,每组60例。其中试验组患儿使用肺表面活性物质剂量为100 mg/kg,对照组患儿使用肺表面活性物质剂量为70mg/kg,比较两组患儿的治疗效果。结果治疗后,试验组的撤机时间为(7.43±1.84)d,明显低于对照组的(11.65±2.32)d,差异有统计学意义(P<0.05);两组患儿动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))明显改善,且试验组患儿PaO_(2)明显高于对照组,PaCO_(2)明显低于对照组,差异有统计学意义(P<0.05);试验组患儿CLD发生率为8.33%(5/60),明显低于对照组的21.67%(13/60),差异有统计学意义(χ^(2)=3.883,P=0.049<0.05);试验组患儿不良反应发生率为5.00%,对照组不良反应发生率为6.67%,差异无统计学意义(P>0.05)。结论在正常治疗的剂量范围内,大剂量肺表面活性物质通过气管内滴入治疗可有效预防低出生体重儿CLD发生,且与低剂量给药比较无明显不良反应。 Objective To investigate efficacy of pulmonary surfactant with different doses in treatment of low birth weight infant for preventing chronic lung disease(CLD).Methods A total of 120 low birth weight infants which needed reatment during January 2019 and December 2020 were selected as the research objects,and were randomly divide it into an experimental group and a control group,with 60 cases in each group.The infans in the experimental group were given pulmonary surfactant of 100 mg/kg,and the infants in the control group were given pulmonary surfactant of 70 mg/kg.The therapeutic effect of the two groups was compared.Results After treatment,the weaning time of the experimental group(7.43±1.84)d was significantly lower than the control group(11.65±2.32)d;the PaO_(2) of the experimental group was significantly higher than that of the control group,while PaCO_(2) was lower than that of the control group(P<0.05),and the incidence rate of CLD of the experimental group was 8.33%(5/60),which was significantly lower than that of the control group 21.67%(13/60);with statistically significant differences(P<0.05).The incidence of adverse drug reactions of the experimental group was 5.00%,and the control group was 6.67%,and the difference was not statistically significant(P>0.05).Conclusion Within the dose range of normal treatment,large doses of pulmonary surfactants can be effectively prevented from the occurrence of CLD in low birth weight infants through intratracheal instillation,and there is no obvious adverse reaction compared with low dose administration.
