摘要
目的探讨基于脉搏波检测的社区老年人动脉硬化指标与身体成分的关系。方法采取便利抽样法选取210例社区老年人作为研究对象,采用身体成分测试仪检测身体成分相关指标,脉搏波分析仪检测动脉硬化相关指标,采用Spearman相关分析及多重线性回归分析方法,分析主动脉脉搏波传导速度、增强指数与身体成分各指标之间的关系。结果主动脉脉搏波传导速度与体质指数、脂肪量、体脂率、内脏脂肪含量、浮肿指数呈正相关(P<0.05),与去脂体质量、肌肉量、四肢骨骼肌含量呈负相关(P<0.05)。增强指数与脂肪量、体脂率、皮下脂肪含量、浮肿指数呈正相关(P<0.05),与去脂体质量、肌肉量、四肢骨骼肌含量、骨骼肌质量指数呈负相关(P<0.05)。校正年龄、性别、血压、心率等因素后,内脏脂肪含量是主动脉脉搏波传导速度的独立危险因素,浮肿指数是增强指数的独立危险因素。结论社区老年人中,内脏脂肪含量、浮肿指数分别是主动脉硬化、外周动脉硬化的独立危险因素。
Objective To explore the relationship between arteriosclerosis detected by pulse wave detector and body composition in the community-dwelling elderly.Method A total of 210 elderly residents(age 60 years or older)in the community were chosen by convenient sampling.Arteriosclerosis was detected by pulse wave detector and body composition was tested by body composition tester.Spearman correlation and multiple linear regression methods were used to analyze the relationship between aortic pulse wave velocity(aPWV),augmentation index 75(AIx 75),and body composition indexes.Result aPWV was positively correlated with BMI,fat mass,body fat percentage,visceral fat content,and edema index(P<0.05),and negatively correlated with fat-free mass,muscle mass,and appendix skeletal muscle mass(P<0.05).AIx 75 was positively correlated with fat mass,body fat percentage,subcutaneous fat mass,and edema index(P<0.05),and negatively correlated with fat mass,muscle mass,appendix skeletal muscle mass,and skeletal mass index(SMI)(P<0.05).After adjusting for age,gender,blood pressure and heart rate,visceral fat content and edema index were independent risk factors for aPWV and AIx 75 respectively.Conclusion Among the community-dwelling elderly,visceral fat content and edema index are independent risk factors for aortic sclerosis and peripheral atherosclerosis respectively.
