摘要
目的:归纳总结侯炜教授中医维持治疗鼻咽癌的用药规律与组方特色。方法:提取中国中医科学院广安门医院门诊电子病历数据,经筛选处理后录入中医传承辅助平台(V2.5),采用频数分析、关联规则、复杂系统熵聚类分析等处理方法,明晰核心药物,总结辨治规律。结果:共纳入方剂218首,用药频数较多的有白花蛇舌草、甘草、牛蒡子、苦地丁、半枝莲,用药药性多偏寒、凉,药味多偏甘、苦,归经多为肺、胃经,挖掘强关联的药物组合20条,新处方6首。结论:侯炜教授中医维持治疗鼻咽癌确立了“阴虚毒蕴”的核心病机,以及“益气养阴以求其本,清热化痰散瘀兼顾其标”的治疗思路,疏补兼施,标本兼顾,补诸脏之亏虚,化癌毒之夙根。
Objective:To summarize the medication rule and prescription characteristics of Professor HOU Wei in the TCM maintenance treatment of nasopharyngeal carcinoma.Methods:The electronic medical record data was extracted from the outpatient clinic of Guang’anmen Hospital,China Academy of Chinese Medical Sciences,and then the data was input into the TCM inheritance assistance platform(V2.5)after screening and processing.Frequency analysis,association rules,complex system entropy clustering analysis and other processing methods were adopted to clarify the core drugs and summarize the rules of differentiation and treatment.Results:A total of 218 prescriptions were included,among which the most frequently used ones were Baihuasheshecao(Hedyotis Diffusa),Gnacao(Licorice Root),Niubangzi(Fructus Arctii),Kudiding(Corydalis Bungeana Turcz.)and Banzhilian(Scutellaria Barbata).Most of the drugs were cold and cool,and the tastes were mainly sweet and bitter.Most of the meridian tropisms were lung and stomach.Totally 20 drug combinations with strong association and six new prescriptions were found Conclusion:Professor HOU Wei established the core pathogenesis of"Yin deficiency and poison accumulation"in TCM maintenance treatment of nasopharyngeal carcinoma and the therapeutic idea is"enriching Qi and nourishing Yin for its root,and clearing heat,eliminating phlegm and dispersing blood stasis for its tip",to remedy the deficiency of the viscera and cure nasopharyngeal carcinoma.
