摘要
目的探讨谷参肠安辅助肠内营养支持治疗对老年胃癌术后患者的疗效。方法选择2016年1月至2019年12月在我院接诊的老年胃癌术后患者60例,随机分为对照组(30例)和观察组(30例)。对照组给予普通肠内营养治疗,观察组给予谷参肠安^(+)普通肠内营养治疗。对比两组患者术后第1天、术后第8天营养状况、免疫功能及炎症反应指标的变化情况。营养状况选择血清白蛋白(ALB)、前白蛋白(PAB)、转铁蛋白(TFN)及血红蛋白(HB)作为评价指标;免疫功能包括细胞免疫与体液免疫,细胞免疫选择CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)作为评价指标,体液免疫选择免疫球蛋白(Ig)A、IgG、IgM作为评价指标;炎症反应选择血清肿瘤坏死因子(TNF)-α、白介素(IL)-1β及C反应蛋白(CRP)作为评价指标。比较两组并发症发生率。结果术后第8天,两组患者营养状况指标ALB、PAB、TFN、HB,免疫功能指标CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、IgA、IgG及IgM水平均较术后第1天显著升高,且各指标水平观察组均显著高于对照组(均P<0.05);而炎症反应指标血清TNF-α、IL-1β及CRP水平均显著降低(均P<0.05),且各指标水平观察组均显著低于对照组(均P<0.05)。观察组并发症发生率显著低于对照组(P<0.05)。结论谷参肠安辅助肠内营养支持治疗可以更加有效地改善老年胃癌术后患者营养状况,增强免疫功能,减轻炎症反应及降低并发症发生率。
Objective To investigate the effect of Gushen Chang’an(谷参肠安) plus auxiliary enteral nutrition on elderly patients with gastric cancer after surgery. Methods A total of 60 elderly patients with gastric cancer after surgery who were admitted to our hospital from January 2016 to December 2019 were analyzed. All patients were randomly divided into control group(30 cases)and observation group(30 cases). Patients in control group were treated with common enteral nutrition,and patients in observation group were treated with Gushen Chang’an plus common enteral nutrition. The changes of indexes of nutritional status,immunological function and inflammation of patients were compared between two groups on 1 d after surgery and 8 d after surgery. Serum albumin(ALB),prealbumin(PAB),transferrin(TFN)and hemoglobin(HB)were selected as the indicators of nutritional status. Immunological functions include cellular immunity and humoral immunity. Cluster of differentiation(CD)3^(+),CD4^(+) and CD4^(+)/CD8^(+) were selected as the evaluation index of cellular immunity,and immunoglobulin(Ig)A,IgG and IgM were selected as the indicators of humoral immunity. Serum tumor necrosis factor(TNF)-α,interleukin(IL)-1β and C-reactive protein(CRP)were selected as the evaluation indexes of inflammation. The incidence of complications were also compared between two groups. Results Compared with 1 d after surgery,the levels of nutritional status indexes serum ALB,PAB,TFN and HB,and immunological function indexes CD3^(+),CD4^(+),CD4^(+)/CD8^(+),IgA,IgG and IgM increased in both groups on 8 d after surgery,and the levels of all above indexes in observation group on 8 d after surgery were significantly higher than that in control group on 8 d after surgery(P<0.05). Compared with 1 d after surgery,the levels of inflammation indexes serum TNF-α,IL-1β and CRP decreased in both groups on 8 d after surgery,and the levels of above three indexes in observation group on 8 d after surgery were significantly lower than that in control group on 8
