2015-2019年某妇产医院NICU新生儿医院感染病原菌耐药分析 被引量:9

Analysis of antibiotic resistance of nosocomial pathogens in NICU neonates of a maternity hospital from 2015 to 2019

摘要 目的探究2015-2019年杭州市妇产科医院NICU新生儿医院感染情况,旨在为临床诊断和治疗NICU新生儿医院感染提供参考。方法选取2015年1月-2019年10月杭州市妇产科医院收治的NICU新生儿2296例为研究的对象,按照入院时间将所有NICU新生儿分为2015年组、2016年组、2017年组、2018年组、2019年组,比较各组NICU新生儿医院感染的一次感染率和二次及以上感染率,统计感染部位、病原菌种类以及多药耐药菌。结果2015-2019年感染率和二次感染率逐渐降低,多药耐药菌数量呈逐年增长的趋势;近5年NICU新生儿感染部位占比由高至低分别为呼吸道、口腔、血液、泌尿道以及其他,病原菌共检测出213株,其中革兰阴性菌100株,革兰阳性菌73株,真菌26株,病毒14株。结论近5年NICU新生儿医院感染发生率呈逐年降低的趋势,感染部位以呼吸道为主,病原菌种类以革兰阴性菌为主,但多药耐药菌检出率逐年上升,临床应加以重视。 OBJECTIVE To explore nosocomial infection status of neonates in NICU from 2015 to 2019,and to provide a scientific basis for clinical diagnosis and treatment of nosocomial infection in NICU neonates.METHODS 2296 cases of critically ill neonates admitted to Hangzhou Obstetrics and Gynecology Hospital from January 2015 to October 2019 were selected as the research subject,and divided into 2015 group,2016 group,2017 group,2018 group,2019 group according to the time of admission.The primary infection rate and the secondary infection rate of nosocomial infections in each group of NICU neonates were compared,and the infection sites,pathogen types and multi-resistant bacteria were counted.RESULTS From 2015 to 2019,the infection rate and secondary infection rate had gradually decreased,and the number of multi-drug resistant bacteria showed a trend of increasing year by year.In the past 5 years,the proportions of infection sites in NICU neonates were respiratory tract,oral cavity,blood,urinary tract,and others.A total of 213 strains of pathogens were detected,including 100 strains of Gram-negative bacteria,73 strains of Gram-positive bacteria,26 strains of fungi and 14 strains of viruses.CONCLUSION The incidence of nosocomial infections in NICU neonates in the past 5 years had been decreasing year by year.The main infection site was the respiratory tract,and the pathogenic bacteria were mainly Gram-negative bacteria.However,the proportion of multi-drug resistant bacteria was increasing year by year,and attention should be paid to the clinic.
