摘要
目的探讨系统性红斑狼疮患者外周血T淋巴细胞亚群和亲环素A(CyP A)的表达及临床意义。方法选取本院34例健康体检者(对照组)及收治的52例系统性红斑狼疮患者(研究组)作为研究对象,后者根据其病情分为活动组和非活动组、狼疮性肾炎组和无狼疮性肾炎组,分别检测86例研究对象的外周血T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、CD4^(+)/CD8^(+)、CyP A及抗CyP A自身抗体的表达情况并加以比较。结果研究组患者T淋巴细胞亚群CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均低于对照组,CD8^(+)和CyP A高于对照组(P<0.05)。在52例系统性红斑狼疮患者中,活动组、狼疮性肾炎组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均低于非活动组、无狼疮性肾炎组,CD8^(+)和CyP A高于非活动组、无狼疮性肾炎组(P<0.05)。对照组抗CyP A抗体的阳性率为2.94%(1/34),低于研究组46.15%(24/52)(P<0.05),三类抗体IgG、IgA、IgM的阳性率分别为36.54%(19/52)、19.23%(10/52)、13.46%(7/52)。抗CyP A抗体在出现皮肤黏膜损害、关节痛、发热、白细胞减少等临床表现的患者中的阳性率较高。结论外周血T淋巴细胞亚群和CyP A的表达水平与系统性红斑狼疮的发生发展、病情程度有密切联系。
Objective To investigate the expression and clinical significance of peripheral blood T lymphocyte subsets and cyclophilin A(CyP A)in patients with systemic lupus erythematosus. Method 34 people with healthy examination(control group)and 52 patients with systemic lupus erythematosus(study group)were enrolled. The latter were divided into active and inactive groups,lupus nephritis group and no-lupus nephritis group according to their condition of desease. The expressions of peripheral blood T lymphocyte subsets(CD3^(^(+)),CD4^(+),CD8^(+)),CD4^(+)/CD8^(+),CyP A and anti-CyP A autoantibodies in 86 subjects were detected and compared. Results The T lymphocyte subsets CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the study group were lower than those in the control group,while CD8^(+)and CyP A were higher than those in the control group(P<0.05).In 52 patients with systemic lupus erythematosus,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the active group and lupus nephritis group were lower than those in the inactive group and no-lupus nephritis group,while CD8^(+)and CyP A were higher than those in the inactive group and no-lupus nephritis group(P<0.05). The positive rate of antiCyP A antibody in the control group was 2.94%(1/34),which was lower than that in the study group (46.15%(24/52)(P<0.05). The positive rates of the three types of antibodies Ig G,Ig A and Ig M were 36.54%(19/52),19.23%(10/52)and 13.46%(7/52)respectively. Anti-Cy P A antibodies had a higher positive rate in patients with clinical manifestations such as damage of skin mucous membrane,arthralgia,fever and leukopenia,etc. Conclusion The expression levels of peripheral blood T lymphocyte subsets and Cy P A are closely related to the occurrence and development of systemic lupus erythematosus and to the degree of disease.
