儿童肺炎支原体肺炎中MP载量的临床意义及可能发病机制 被引量:14

摘要 目的探讨肺炎支原体肺炎(MPP)患儿肺泡灌洗液(BALF)中MP载量的临床意义,探索MP感染的发病机制。方法选取确诊为MPP并行支气管镜检查的患儿为MPP组,按BALF中MP的载量不同分为高MP组和低MP组;同期因支气管异物行支气管镜的儿童作为对照组。用实时荧光定量PCR(RT-qPCR)法检测CARDS TX的表达量,分析比较组间患儿的临床特征、实验室检查及MP载量与CARDS TX的关系。结果高MP组患儿热程、热峰比低MP组更长、更高,且高MP组发生胸腔积液、肺实变、肺不张的比例更高。低MP组WBC计数的平均值高于高MP组及对照组;高MP组和低MP组NE、CRP高于对照组,且高MP组CRP更高;高MP组LYM、EOS低于低MP组、对照组。高MP组CD_(3)^(+)、CD_(3)^(+)CD_(4)^(+)、CD_(3)^(-)CD_((16+56))^(+)、CD_(3)^(-)CD_(19)^(+)均低于低MP组。MPP组CARDS TX的表达量高于对照组,且高MP组更高。结论CARDS TX的表达量与MP载量有关,高载量MP患儿炎症反应更重,细胞免疫功能紊乱更明显。 Objective To explore the clinical significance of MP load in BALF of children with Mycoplasma pneumoniae pneumonia(MPP)and to explore the pathogenesis of MP infection.Methods The children who were diagnosed as MPP and underwent bronchoscopy were selected as the MPP group and divided into the high MP group and the low MP group according to the load of MP in BALF,and the children who underwent bronchoscopy because of bronchial foreign bodies during the same period were taken as the control group.The expression of CARDS TX was detected by RT-qPCR method,and the clinical features,laboratory examination and the correlation between MP load and CARDSTX were analyzed and compared.Results The heat duration and thermal peak in the high MP group were longer and higher than those in the low MP group,and the proportion of pleural effusion,pulmonary consolidation and atelectasis was higher in the high MP group.The average value of WBC count in the low MP group was higher than that in the high MP group and the control group.NE and CRP in the high MP group and the low MP group were higher than those in the control group,and CRP in the high MP group was higher.LYM and EOS in the high MP group were lower than those in the low MP group and the control group.CD_(3)^(+),CD_(3)^(+)CD_(4)^(+),CD_(3)^(-)CD_((16+56))^(+) and CD_(3)^(-)CD_(19)^(+) in the high MP group were lower than those in the low MP group.The expression of CARDS TX in the MPP group was higher than that in the control group,and higher in the high MP group.Conclusion The expression of CARDSTX is related to the load of MP.The inflammatory reaction is more severe and the disorder of cellular immune function is more obvious in children with high load of MP.
