高胆红素血症早期新生儿脑功能变化观察 被引量:6

摘要 目的观察高胆红素血症早期新生儿的脑功能变化。方法58例高胆红素血症患儿,均于入院后行振幅整合脑电图(aEEG)检测,分析脑电背景活动周期性指标[3 h内的睡眠—觉醒周期(SWC)数量、单个SWC持续时间、下边界电压、带宽、窄带宽、窄带时长占比]变化。抽取外周静脉血检测血清总胆红素、白蛋白,测算血清总胆红素与白蛋白比值(B/A)。结果58例患儿中3 h内SWC数量≤2组22例(A组)和3 h内SWC数量>2组36例(B组)。A组下边界电压(5.43±1.43)μV、带宽(24.58±7.73)μV、窄带宽(7.00±1.94)μV,1个SWC中窄带时长占比0.71±0.05;B组下边界电压(5.64±1.15)μV、带宽(25.92±7.72)μV、窄带宽(7.17±2.32)μV,1个SWC中窄带时长占比0.64±0.72。与B组比较,A组1个SWC中窄带时长占比高(P<0.05)。A组血清总胆红素峰值(402.92±43.03)μmol/m L,血清白蛋白(36.92±2.10)g/L,B/A值4.40±0.74;B组血清总胆红素峰值(378.13±29.35)μmol/m L,血清白蛋白(36.47±2.97)g/L,B/A值6.10±0.64;与B组比较,A组血清总胆红素水平升高(P<0.05)。结论高胆红素血症新生儿早期即出现脑电背景活动周期性改变。高胆红素血症早期即可造成新生儿睡眠功能变化。 Objective To observe the changes of brain function in neonates with hyperbilirubinemia in the early stage.MethodsAmplitude-integrated electroencephalogram(a EEG)was performed in 58 children with hyperbilirubinemia after admission.The changes of a EEG background activity periodic indexes[number of sleep-wake cycles(SWC)within 3 h,duration of single SWC,lower boundary voltage,bandwidth,narrow bandwidth,and proportion of narrow bandwidth duration]were analyzed.Peripheral venous blood of the children was collected to detect serum total bilirubin and albumin,and we calculated the ratio of serum total bilirubin to albumin(B/A).ResultsAmong the58 children,there were 22 cases in the group A with SWC less than or equal to 2 within 3 h,and 36 cases in the group B with SWC more than 2 within 3 h.The lower boundary voltage,bandwidth,narrow bandwidth,and the proportion of narrow band time in one SWC of the group A were(5.43±1.43)μV,(24.58±7.73)μV,(7.00±1.94)μV and 0.71±0.05.The lower boundary voltage,bandwidth,narrow bandwidth and the proportion of narrow band time in one SWC of group B were(5.64±1.15)μV,(25.92±7.72)μV,(7.17±2.32)μV and 0.64±0.72.Compared with group B,the proportion of narrow band duration in one SWC in the group A was higher(P<0.05).In the group A,the peak value of serum total bilirubin was(402.92±43.03)μmol/m L,serum albumin was(36.92±2.10)g/L,B/A value was 4.40±0.74;in the group B,the peak value of serum total bilirubin was(378.13±29.35)μmol/m L,serum albumin was(36.47±2.97)g/L,B/A value was 6.10±0.64.Compared with group B,the level of serum total bilirubin in group A increased(P<0.05).ConclusionsThe periodic changes of a EEG background activity occurred in the early stage of neonates with hyperbilirubinemia.The sleep function of neonatal brain tissues may change at the early stage of hyperbilirubinemia.
