摘要
目的:探讨经外周静脉穿刺的中心静脉导管(Peripherally inserted central venous catheter,PICC)置管后的动态干预预防血栓形成的效果。方法:选取接受PICC置管患者80例,按照随机数字表法分为对照组与观察组,各40例。对照组接受常规护理,观察组接受临床预防血栓路径干预措施,比较两组干预前后凝血功能指标、导管相关性血栓发生率和护理满意度。结果:观察组干预后凝血酶原时间(PT)(20.14±3.68)s、活化部分凝血活酶时间(APTT)(40.36±6.32)s、凝血酶原时间-国际标准化指数(INR)(1.91±0.25)均长于对照组[(16.22±3.15)s、(34.40±5.71)s、(1.63±0.22)],差异均有统计学意义(P均<0.05);观察组干预后血栓发生率(5.00%)低于对照组(20.00%),护理满意度(95%)高于对照组(80%),差异均有统计学意义(P均<0.05)。结论:临床路径根据置管后预防血栓的形成进行动态干预,干预的实施可有效改善PICC置管后凝血指标,减少导管相关性血栓的发生,提升患者护理满意度。
Objective:To investigate the effect of dynamic intervention to prevent thrombosis after PICC catheterization.Methods:80 patients receiving PICC catheterization were selected and divided into control group and observation group according to the dynamic nursing method of random number table,40 patients each.The control group received routine nursing,and the observation group received clinical intervention measures to prevent thrombus path.The indicators of coagulation function,incidence of catheter-related thrombosis and nursing satisfaction were compared between the two groups before and after intervention.Results:After intervention,prothrombin time(PT)(20.14±3.68)s,activated partial thrombin time(APTT)(40.36±6.32)s,and prothrombin time(INR)(1.91±0.25)in the observation group were longer than those in the control group[(16.22±3.15)s、(34.40±5.71)s、(1.63±0.22)],and the differences were statistically significant(P<0.05).The incidence of thrombosis after intervention in the observation group(5.00%)was lower than that of the control group(20.00%),nursing satisfaction(95%)was higher than that of the control group(80%),the difference was statistically significant(P<0.05).Conclusion:The clinical pathway is dynamically intervened according to the prevention of thrombosis after catheterization.The implementation of intervention can effectively improve the coagulation index after PICC catheterization,reduce the occurrence of catheter-related thrombosis,and improve the nursing satisfaction of patients.
