摘要
目的探讨电化学发光免疫法(ECLIA法)、酶联免疫法(ELISA法)在梅毒诊断筛查中的应用价值.方法回顾性分析2016年2月~2020年9月在本检测中心进行梅毒感染测定的186例受检者,所有受检者均接受ECLIA法、ELISA法及梅毒螺旋体颗粒凝集试验(TPPA)检测,并以TPPA检测结果作为诊断金标准,分析ECLIA法、ELISA法在梅毒筛查诊断中的价值.结果186例受检者中TPPA检测结果显示梅毒阳性为111例,阴性为75例;其中ECLIA法真阳性109例,真阴性65例;灵敏度98.20%(109/111)、特异度86.67%(65/75),准确度93.55%(174/186).ELISA法真阳性105例,真阴性71例;灵敏度94.59%(105/111)、特异度94.67%(71/75),准确度94.62%(176/186).两组患者的灵敏度对比(x^(2)=2.075)、特异度对比(x^(2)=2.836)、准确度对比(x^(2)=0.193),差异无统计学意义(P>0.05).结论ECLIA法、ELISA法在梅毒的诊断筛查中均具有较高的应用价值,两种测定方式灵敏度、特异度、准确度均处于较高水平,其中ECLIA法灵敏度稍高于ELISA法,在筛查时可减少漏诊的发生.
Objective To explore the application value of electrochemiluminescence immunoassay(ECLIA)and enzyme-linked immunoassay(ELISA)in syphilis diagnosis and screening.Me thods A retrospective analysis was conducted on 186 subjects who underwent syphilis infection detection in this testing center from February 2015 to September 2020.All subjects were tested by ECLIA,ELISA and Treponema pallidum particle agglutination test(TPPA),and the results of TPPA were used as the diagnostic gold standard to analyze the value of ECLIA and ELISA in syphilis screening and diagnosis.Results TPPA test results of 186 subjects showed 111 syphilis positive and 75 syphilis negative,including 109 true positive and 65 true negative cases by ECLIA method;sensitivity 98.20%(109/111),specificity 86.67%(65/75),accuracy 93.55%(174/186).There were 105 true positives and 71 true negatives by ELISA;the sensitivity was 94.59%(105/111),specificity was 94.67%(71/75),and accuracy was 94.62%(176/186).There were no significant differences in sensitivity(x^(2)=2.075),specificity(x^(2)=2.836),accuracy(x^(2)=0.193)between the two groups(P>0.05).Conelusion ECLIA method and ELISA method have high application value in the diagnosis and screening of syphilis.The sensitivity,specificity and accuracy of the two methods are at a higher level,among which the sensitivity of ECLIA method is slightly higher than that of ELISA method,which can reduce the occurrence of missed diagnosis in screening.
