基于脑-肢协同调控模式探讨LF-rTMS联合推拿对脑卒中后肢体运动功能障碍的影响 被引量:16

Effect of LF-rTMS Combined with Tuina on Post-Stroke Motor Dysfunction of Limbs Based on Brain-Limb Coordinated Regulation Model

摘要 目的:观察低频重复经颅磁刺激(Low Frequency Repetitive Transcranial Magnetic Stimulation,LF-rTMS)联合推拿治疗对脑卒中后肢体运动功能障碍的疗效。方法:将80例符合纳入标准的脑卒中后肢体运动功能障碍患者采用随机数字表分成治疗组和对照组,两组均予以常规药物治疗和康复训练,对照组在此基础上加用LF-rTMS治疗,治疗组在对照组基础上辅以患肢推拿治疗,分别于治疗前、治疗2周后及治疗4周后采用Fug1-Meyer运动功能评分(Fugl-Meyer Assessment,FMA)、改良Barthel指数(Modified Barthel Index,MBI)评分、握力测量仪对患者进行疗效评估。结果:两组治疗前FMA评分、MBI评分及握力值比较均无统计学意义(P>0.05);治疗2周后,两组患者FMA、MBI评分及握力值较治疗前均改善(P<0.05),治疗组在FMA和MBI评分方面优于对照组(P<0.05);治疗4周后,两组患者FMA、MBI评分及握力值均较治疗前进一步改善(P<0.05),且治疗组上述各项指标优于对照组(P<0.05)。结论:LF-rTMS联合患肢推拿能有效促进脑卒中患者肢体运动功能恢复,且具有协同作用,疗效更佳,值得临床推广。 Objective:To observe the effect of low-frequency repetitive transcranial magnetic stimulation(LF-rTMS)combined with Tuina on post-stroke motor dysfunction of limbs.Methods:80 patients with post-stroke motor dysfunction of limbs,who met the inclusion criteria were randomly divided into the treatment group and the control group.Both groups were treated with conventional medication and rehabilitation training;on which basis,the control group was intervened with LF-rTMS;whereas the treatment group was also treated with Tuina therapy for affected limbs on the treatment basis of the control group.The scores of Fug1-Meyer Assessment(FMA)and Modified Barthel Index(MBI),as well as the measurement of grip strength were observed and compared before the treatment,after 2 weeks of the treatment and after 4 weeks of the treatment in the two groups.Results:There were no statistical differences in FMA score,MBI score and grip strength between the two groups before the treatment(P>0.05);which were all significantly improved in the two groups after 2 weeks of the treatment,and the improvements in the treatment group were more significant(P<0.05).After 4 weeks of the treatment,FMA score,MBI score and grip strength were further improved compared to those before the treatment in the two groups(P<0.05),and the improvements in the treatment group were more significant than those in the control group(P<0.05).Conclusion:LF-rTMS combined with Tuina can effectively promote the recovery of motor function in the stroke patients,which has a synergistic effect and is worthy of clinical promotion.
