摘要
目的观察分析疼痛综合干预对腹腔镜胆囊切除术康复效果的影响,进一步确定该护理干预方法的价值。方法选取2018年5月至2020年5月收治的行腹腔镜胆囊切除术的患者80例作为本次研究对象,采用随机数字表法分为对照组和试验组,每组40例。对照组行常规护理,试验组行疼痛综合护理干预。观察并比较2组患者术后疼痛评分、焦虑和抑郁情况。2组患者术后并发症出现情况进行观察记录,主要出现胆漏、切口感染、不良胃肠道反应和尿潴留情况。结果2组患者术后6 h疼痛评分差异无统计学意义(P>0.05),但术后24 h、48 h、72 h的疼痛评分,试验组均低于对照组,差异有统计学意义(P<0.05);护理干预前2组患者SAS、SDS评分比较差异无统计学意义(P>0.05),护理干预后,试验组患者SAS、SDS评分明显低于对照组,差异有统计学意义(P<0.05);护理后,试验组SAS、SDS评分低于护理前,生活质量各维度分高于护理前,差异有统计学意义(P<0.05);对照组护理干预后GH、RP、RE、SF、BP、SF、VT、MH方面的积分高于护理干预前,差异有统计学意义(P<0.05),与对照组比较,试验组在改善GH、RP、RE、SF、BP、SF、VT、MH方面优于对照组,差异有统计学意义(P<0.05);试验组患者并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论对进行腹腔镜胆囊切除术患者采用疼痛综合干预措施可有效减轻患者的疼痛程度,改善患者不良心理,降低并发症发生率,有较高的临床推广价值。
Objective To investigate the rehabilitation effects of pain comprehensive nursing intervention for the patients after laparoscopic cholecystectomy.Methods A total 80 patients who underwent laparoscopic cholecystectomy in our hospital from May 2018 to May 2020 were enrolled in the stdy,who were were divided into control group and experimental group according to random number table method,with 40 cases in each group.The patients in control group were treated by routine nursing,however,the patients in experimental group were treated by pain comprehensive nursing intervention.The postoperative pain score,anxiety and depression status,and the incidence of postoperative complications,including bile leakage,incision infection,adverse gastrointestinal reactions and urinary retention were observed and compared between the two groups.Results There were no significant differences in pain scores at 6h after operation between the two groups(P>0.05),however,which at 24h,48h and 72h in experimental group were significantly lower than those in control group(P<0.05).Before nursing intervention,there were no significant differences in SAS and SDS scores between the two groups(P>0.05).After nursing intervention,SAS scores and SDS scores in experimental group were significantly lower than those in control group and those before nursing(P<0.05),and the scores of all dimensions of life quality in experimental group were significantly higher than those before nursing(P<0.05).After nursing intervention,the scores of GH,RP,Re,SF,BP,SF,VT,MH in control group were significantly higher than those before nursing(P<0.05),moreover,which in experimental group were superior to those in control group(P<0.05).In addition the incidence rate of complications in experimental group was significantly lower than that in control group(P<0.05).Conclusion The pain comprehensive nursing intervention for the patients undrgoing laparoscopic cholecystectomy can effectively relieve the pain of patients,improve the adverse psychological status,decrease the incid
