摘要
目的探讨高频超声联合腹部彩超在下肢静脉血栓诊断中的应用价值。方法该研究选取2017年1月—2019年12月该院收治的319例疑似下肢静脉血栓患者作为研究对象,所有患者均接受CT静脉造影检查,超声阳性患者进一步进行下肢深静脉造影检查,对其临床资料进行回顾性分析。结果该组319例疑似下肢静脉血栓患者超声共检出203例下肢静脉血栓,总检出率为63.64%(203/319);203例下肢静脉血栓患者中共计267支静脉发现血栓;对203例超声诊断为下肢静脉血栓的患者进一步进行CT静脉造影检查,共检出83例下肢静脉血栓,总检出率为40.89%(83/203);83例下肢静脉血栓患者中共计96支静脉发现血栓。高频超声联合腹部彩超用于诊断下肢静脉血栓的准确性为31.09%(83/267),敏感性为100.00%(83/83);高频超声联合腹部彩超用于诊断不同部位下肢静脉血栓的准确性为35.96%(96/267),敏感性为100.00%(96/96);高频超声联合腹部彩超用于诊断侧支循环形成的准确性为100.00%(116/116),敏感性为100.00%(116/116)。结论高频超声联合腹部彩超作为下肢深静脉血栓检查的重要手段,用于诊断下肢静脉血栓具有一定应用价值,将彩超检查作为下肢深静脉血栓的唯一诊断标准仍需进一步大样本数据支持。
Objective To explore the application value of high-frequency ultrasound combined with abdominal color Doppler ultrasound in the diagnosis of lower extremity venous thrombosis.Methods In this study,319 patients with suspected lower limb venous thrombosis admitted to the hospital from January 2017 to December 2019 were selected as the research subjects.All patients underwent CT venograpgy.Patients with positive ultrasound were further subjected to deep venography of the lower limbs.Retrospective analysed of its clinical data.Results In this group of 319 patients with suspected lower extremity venous thrombosis,203 cases of lower extremity venous thrombosis were detected by ultrasound,with a total detection rate of 63.64%(203/319);a total of 267 vein thrombosis were found in 203 cases of lower extremity venous thrombosis;a total of 83 patients with lower extremity venous thrombosis were detected by CT venography of the 203 cases,and the total detection rate was 40.89%(83/203);a total of 96 veins were found in 83 patients with lower extremity venous thrombosis.The accuracy of high-frequency ultrasound combined with abdominal color Doppler ultrasound for diagnosing lower extremity venous thrombosis was 31.09%(83/267),and the sensitivity was 100.00%(83/83);high-frequency ultrasound combined with abdominal color Doppler ultrasound was used to diagnose lower limb veins in different parts,the accuracy of thrombus was 35.96%(96/267),and the sensitivity was 100.00%(96/96);the accuracy of high-frequency ultrasound combined with abdominal color Doppler ultrasound for diagnosing collateral circulation was 100.00%(116/116),the sensitivity was 100.00%(116/116).Conclusion High-frequency ultrasound combined with abdominal color Doppler ultrasound as an important method for the diagnosis of lower extremity deep vein thrombosis has certain application value,the color Doppler ultrasound examination as the only diagnostic criterion for lower extremity deep vein thrombosis still needs further large sample data support.
